Job Description
Purpose:
The primary purpose of your job description is to provide direct nursing care to the residents and to supervise the day-to-day nursing activities performed by nursing assistants and licensed practical nurses. Such supervision must be in accordance with your independent judgment, current federal, state and local standards, Florida RN Nurse Practice Act and regulations that govern our facility and as may be required by the Director of Nursing Services or Nurse Supervisor to ensure that the highest degree of quality of care and employee performance is maintained at all times.
Delegation of Authority:
As a RN you are delegated the administrative authority, responsibility and accountability necessary for carrying out your assigned duties.

The following duties, responsibilities and functions have been determined by Hampton Court Nursing and Rehabilitation Center to be essential to the successful performance of this position.
Duties, Responsibilities and Functions
RESIDENT INTERACTION
- Talks to and interacts with residents respectfully; be knowledgeable of and sensitive to residents verbal and nonverbal communication.
- Provide guidance, counseling and assistance in order to engage residents in ways that are relevant, purposeful and in tune with resident choices, interests and concerns.
- Guide and assist residents in the activity of daily living needs such as bathing, dressing, feeding, transfers, etc.
- Be attentive to and respect residents’ desires and cues for personal space and privacy.
- Interact professionally and appropriately with residents in regard to attitude, self-control, self-discipline, cooperation, and tone of voice, language and usage and in other areas as needed.
- Work with residents to discover and pursue personal goals in keeping with their risk management plans and care plans.
- Serve as a resident advocate by communicating effectively, incorporating resident values in plan of care, facilitating resident decision making and honoring and respecting resident rights and choice.
GENERAL FUNCTIONS, ROLES, RESPONSIBILITIES AND FUNCTIONS
- Conduct the daily nursing functions in accordance with your independent judgment, current rules, regulations, and guidelines that govern the facility.
- Ensure that all nursing personnel assigned to you comply with the written policies and procedures including the safety program.
- Make rounds with health care provider.
- Complete daily clinical and environmental rounds to evaluate resident care being provided and report significant events.
- Meet and resolve issues with residents and/or family members. Report significant issues to your supervisor.
- May be responsible for clinical programs as directed, e.g. dementia, wound care, quality assurance, infection control, admissions.
- Communicate with health care provider to review treatment plans, complete orders, progress notes, etc., in accordance with established policies.
- Review the resident’s medical record for specific treatments, medication orders, diets, labs etc., as for implementation and follow up.
- Communicate with written (24 hour report) and oral reports/recommendations concerning any changes in resident condition and other activities of your shift to nursing management.
- Fill out and complete accident/incidents involving residents or staff and submit to Director of Nursing.
- Complete and file assessments and all other medical record documentation as required to provide care services.
- Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the supervisor.
- Complete clinically accurate documentation according to policy.
- Review the resident medical documentation and progress notes from the interdisciplinary team to determine if the care plan is being followed. Report any concerns to your supervisor.
- Notify the resident’s attending health care practitioner and family when there is a change in the resident’s condition.
- Meet with your assigned support personnel (Housekeeping, Dining Services, Nursing Assistants, etc.) to plan the shift’s services, programs, and activities and monitor tasks are completed as directed.
- Coordinate services with other members of the disciplinary team (Housekeeping, Dining Services, Nursing, etc.) to ensure that the resident’s total regimen of care is maintained.
- Assist in arranging transportation and appointments for residents and other third party services.
- Review medication administration record for completeness of information and accuracy.
- Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand during your shift.
- Adjust the staffing needs of the nursing service department necessary to meet the total nursing needs of the residents.
- Develop work assignments, schedule duty hours, and assist and supervise nursing staff to ensure that all personnel are performing their work assignments in accordance with acceptable nursing standards.
- Assist in staff development. Formally and informally monitor performance of personnel and ensure adjustments/corrections are made by using coaching, counseling, and discipline methods.
- Work with the Director of Nursing to resolve staff performance issues including those that may lead to termination.
- Review complaints and grievances, complete investigations, and work with the Director of Nursing for a plan of action and resolution.
- Participate in the planning, conducting and scheduling of timely in service and orientation training classes that meet state and federal requirements.
- Maintain nursing service work areas (i.e., nurses’ stations, medicine preparation rooms, etc.), resident rooms, and treatment areas in a clean and sanitary manner.
- Meet with and resolve personnel issues with the licensed practical nurses who you supervise.
- Recommend to the Director of Nursing the equipment and supply needs of the nursing department. Ensure that a stock level of medications, medical supplies, equipment, etc., is maintained on the premise at all times to adequately meet the needs of the residents.
- Integrate advanced knowledge and experience in delivering safe, effective quality care to the residents.
- Demonstrates competence in managing health / illness status of residents.
- Manage and negotiate within the healthcare delivery system on behalf of the residents.
- Monitor and ensure quality healthcare for the residents.
- Incorporate an understanding of trends in aging in planning and providing care.
- Demonstrate leadership and competence in implementing the role of an RN.
- Engage in counseling, communication, collaboration and teaching in a manner that reflects caring, advocacy, ethics, and professional standards.
- Understand patient acute and chronic illness and react accordingly.
- Interpret and evaluates diagnostic tests to identify and assess patient condition. Develops a plan and discusses it with the physician or designee.
- Consults and coordinates with health care team members to assess, plan, implement and evaluate patient care.
- Modifies patient treatment plan as indicated by patient responses and conditions.
- When dealing with ill or injured residents you are knowledgeable and proficient in your duties in managing IVs, ensuring residents and their family members understand the resident’s condition and treatments, supervise nursing staff, monitor fluid and food intake and output and move residents safely.
- Observe residents, chart findings, take necessary actions and report changes in patient condition in an efficient, effective and accurate manner.
- Provide resident care or treatments such as performing and recording temperature, pulse, respirations, blood pressures, dressing skin impairments, treating skin impairments, giving enemas, suctioning, inserting indwelling catheters, etc.
- Assemble and use equipment safely and accurately under the standards of nursing for catheters, trach tubes, oxygen, supplies, etc.
- Evaluate nursing intervention outcomes and confers with other health care team members as necessary.
- Observe, receive and obtain information from relevant sources to complete tasks through collection of clinical data and obtaining information from other staff members, family members, residents, vendors, physicians, etc.
- Provide information, analyze and evaluate results to choose the best solution and solve problems in making decisions as it relates to your duties, responsibilities, roles and functions.
- Provide information to supervisors, coworkers, physicians, etc. by telephone, in written form or in person in an accurate and timely manner.
- Identify information by categorizing, estimating, recognizing differences or similarities and detecting changes in circumstances or events.
- Identify body response variations, understand properties of medications and understand technical operating of equipment.
- Is knowledgeable and reacts within licensure standards / facility policies and procedures related to: medical emergencies, hazardous materials, infection control practices, accident care, interpersonal communication techniques, nursing practices, supervision of nursing assistants and licensed practical nurses, resident observation, sanitation practices, basic first aid principles, medication management, medical record documentation, medical terminology and nursing terminology.
- Monitors and reviews information from materials, events or the environment to detect or assess problems and identifies and intervenes in a solution.

- Utilizing independent judgment, evaluate the skills of your licensed practical nurses and the needs of your residents and assign your subordinate employees to residents following this ongoing evaluation process.
- Supervise the day-to-day activities of your licensed practical nurses and oversee their supervision of their nursing assistants.
- Direct your subordinate employees in the performance of their job duties and discipline them when deemed appropriate by you.
- Analyze data or information by identifying the underlying principles, reasons or facts of information by breaking down information or data.
- Demonstrate on a consistent basis the knowledge and performance of unit rounds and nursing documentation according to facility policies and procedures and nursing standards.
- Demonstrate understanding of medication and pharmacy policies and procedures.
- Demonstrate knowledge of the principles of pain management through identification and documentation.
- Demonstrate knowledge of the discharge process through identification and documentation.
- As a mandated reporter, identifies, reports and informs appropriate professionals of Vulnerable Adult issues.
- Adhere to medical orders and directions from medical staff and supervisors concerning resident health issues, treatments and therapies and is attentive to details in the orders and directions.
- Discuss with medical personnel by phone regarding resident health issues accurately and timely.
- Safely administer medications according to the med pass policy and procedure.
- Maintain thorough and legible professional documentation of medical issues in resident chart.
- Attend staff meetings and other meetings as requested by the facility and lead such meetings where appropriate, and maintains license on a current active status.
- Complete annual training and education requirements required by the facility.
- Understand and uphold Hampton Court Nursing and Rehab Center’s values, ethics, moral and corporate compliance standards at all times regardless of personal practices and beliefs.
- Maintain confidentiality and professionalism regarding all facility information regarding resident, staff and facility issues. This also means refraining from discussing resident personal issues in front of residents.
- You must be punctual and ready to start your shift as designated times and you must continue working until you are replaced by another staff person.

- Participate in the development, maintenance and implementation of the facility’s quality assurance and performance improvement program.
- Knowledgeable and participates in facility surveys and inspections made by authorized government agencies in an efficient and effective manner.
- Reviews and updates residents written discharge plan, recapulation of stay as required.
- Properly and accurately relays facility policies and procedures to personnel, residents, visitors and government agencies as required.
- Admit, transfer and discharge residents as required by facility policies and procedures and nursing standards.
- Complete accident/incident/unusual occurrence reports as required timely and accurately.
- Perform duties such as completing and reporting medical forms, reports, evaluations, studies and charting, reviewing xray and lab reports accurately and timely.
- Complete and file required recordkeeping forms/charts upon resident admission, discharge, transfer or during daily care.
- Receive telephone orders from physicians and record on the physician order form completing all sections.
- Transcribe orders timely, accurately to the resident charts, medication record, treatment record, care plans, lab book, etc and fax orders accordingly.
- Document nurses notes in an informative and descriptive manner that reflects the care provided to the resident as well as the resident’s response to the care.
- Report all discrepancies noted concerning physician orders, diet change, charting errors, etc to your supervisor.
- Fill out and complete transfer forms in accordance with established policies and procedures.
- Ensure that appropriate documentation concerning unauthorized discharges is entered in the resident’s medical record in accordance with facility policies and procedures.
- Perform routine charting duties as required in accordance with established charting and documentation policies and procedures.
- Perform employee performance evaluations concerning licensed practical nurses supervised by you.
- Interpret the facility’s policies and procedures for subordinate employees.
- Consult with the resident’s physician in providing and coordinating resident care and services, treatments, etc., as necessary.
- Review resident charts for specific treatments, consult recommendations, medication orders, diets, etc as necessary.
- Implement and maintain established nursing objectives and standards.
- Make periodic checks to ensure that prescribed treatments and nursing standards are being properly administered by the LPNs and CNAs and to evaluate the resident’s emotional and physical status.
WORKING CONDITIONS
- Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants and hazardous chemicals.
- Works throughout the facility.
- Moves intermittently during working hours.
- Is subject to frequent interruptions.
- Is involved with residents, personnel, visitors, government agencies, etc., under all conditions and circumstances.
- Is subject to hostile and emotionally upset residents, family members, personnel and visitors.
- Communicates with the medical staff, nursing personnel and other department staff.
- Works beyond normal working hours and in other positions temporarily when necessary.
- Is subject to call-back during emergency conditions.
- Attends and participates in continuing educational programs.
- Is subject to injury from falls, burns from equipment, odors, etc throughout the work day as well as to reactions from dust, disinfectants, and other air contaminants.
EDUCATION
Must possess a RN license from the state of Florida.
SPECIFIC REQUIREMENTS
- Must possess a current, unencumbered active license to practice as a RN in Florida.
- Must be able to read, write, speak and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such actions.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies and the general public.
- Must be knowledgeable of nursing and medical practices and procedures as well as laws, regulations and guidelines pertaining to long-term care.
- Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
- Must possess the ability to plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures, etc that are necessary for proving quality care.
- Must be able to anticipate the needs of others.
- Must have a cheerful disposition and enthusiasm as well as willingness to handle difficult situations.
- Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
- Must be able to relate information concerning a resident’s condition.
