Program Overview:
The Stabilization Housing Program (SHP) is part of the larger city-funded adult non-congregate shelter system and provides short term supportive housing services for homeless individuals living with HIV/AIDS in San Francisco. The program provides stabilization hotel rooms, client-centered on-site services, and case co-ordination with the goal of stabilizing clients’ physical and mental health while increasing the possibility of future housing stability. This program operates in a 51-unit SRO building in the Tenderloin with 21 units dedicated to SHP.
Position Description:
- Provide short term case management services to currently homeless individuals who are challenged with complex medical issues, mental health issues, and substance abuse issues.
- Under the direction and supervision of the Program Manager, the Case Manager is responsible for assisting clients in the Stabilization Housing Program with linkage to housing resources, light counseling, crisis intervention, and coordination with referring agency case managers, advocacy, information, and referrals to offsite providers.
- Work with on-site Program Coordinator to support a safe and healthy community living environment.
Case Manager Duties:
- Provide crisis intervention and support addressing program violations, conflict and behavioral management, and other assistance as needed.
- Maintain client documentation and records. Assist clients with following their care plans.
- Observe client confidentiality and HIPAA protocols.
- Maintain ARIES database records of client case load and maintain the program’s standard of case management documentation: keep accurate records, files, correspondence, and statistics.
- Enter client and service information into the ONE System.
- Assist clients in applying for permanent housing through Coordinated Entry and other available programs.
- Serve as the on-site support services liaison to staff of referring agencies.
- Maintain referral forms, receive referrals from referring agencies, and review all referrals for accuracy and completion.
- Maintain a strength-based perspective.
- Treat others with respect and courtesy, striving for open and honest working relationships.
- Demonstrate good judgment and common sense.
- Prepare reports and presentations as requested and required.
- Work in collaboration with other onsite staff to facilitate community relations and provide guidance as needed.
- Attend and participate in participant, program, and agency meetings and trainings as assigned.
- Evening hours may be required to assist with events from time to time.
- Other duties as assigned by the Program Manager.
Qualification:

- Bachelor’s degree and 2 years’ experience in social work, preferably in the non-profit or public sector; or equivalent combination of education and experience. Bachelor’s Degree in Social Work or related field preferred.
- Demonstrated ability to work with low-income adults with empathy for persons who are homeless or have a history of homelessness.
- Demonstrated experience in addressing the areas of substance abuse, chronic mental and physical health issues in the homeless population.
- Experience working with people living with HIV/AIDS.
- Ability to work in a fast-paced environment that requires strong multi-tasking capabilities.
- Experience with using Motivational Interviewing and Harm Reduction counseling techniques preferred.
- Knowledge and experience in working with culturally diverse populations.
- Experience in accessing community-based services and ability to relate professionally to other service providers.
- Experience providing social services in a residential setting preferred.
- Excellent writing and analytic skills.
- Able to work independently, make good decisions, and utilize supervision as needed.
- Ability to work collaboratively in a team setting.
- Literacy and experience in using computer software such as Web Browsers, MS Outlook/Word/Excel, and other database programs.
- Bilingual – English/Spanish conversational/written/reading required.
