Alliance Homecare has an established reputation as the premier long-term home care provider in the New York Metropolitan market that sets the highest level of standard within the industry. We are now looking to expand our elite nursing services within in-home care for high-net-worth and ultra-high-net-worth families.
Under the direction of the Director of Operations, the RNCM plays a pivotal role in delivering optimal clinical care and oversight to our patients. Acting as the central point of contact—or "quarterback"—for the patient and their family, the RNCM ensures that the individual needs of each patient are thoroughly assessed and met.
The RNCM must coordinate and communicate effectively with all stakeholders, including the company’s employees, healthcare providers, administrative staff, and key organizational contributors, fostering an environment of trust and respect. Their responsibilities also include the meticulous planning, implementation, coordination, monitoring, and evaluation of care management strategies, ensuring the delivery of exceptional customer service at all times.
In addition, the RNCM contributes to our sales initiatives by enhancing the value and excellence of the Alliance Homecare product and brand, demonstrating a commitment to clinical distinction and operational excellence. Their dedicated engagement in the collaborative process of care management ensures that our patients receive comprehensive and compassionate care tailored to meet their physical and psychosocial needs.
Current geography will include the NYC metro area.
QUALIFICATIONS:
- Bachelors of Science required, Master’s degree preferred.
- Active, unencumbered nursing licensed in the state of New York.
- Excellent analytical and problem-solving skills.
- Effective communication, organizational, and interpersonal skills.
- Strong writing skills, capability to succinctly document all activities with complete nursing notes in secure online charting system.
- Ability to converse effectively with high profile, high-net worth clients by demonstrating concise and carefully articulated verbal and written communication.
- Maintain positive, respectful, and confident body language as well as through placing importance on personal attire, while holding individuals right to privacy to the highest standard and regard with every interaction.

RESPONSIBILITIES:
- Works independently, self-directed, and flexible.
- Serves as the contact, advocate and resource person for clients, families, providers and community partners.
- Evaluates, identifies and effectively coordinates clinical resources and community resources to meet needs of clients/families.
- Collaborates with and works under the direction of the Director of Operations and other team members in the clients’ healthcare in overcoming barriers in meeting goals and objectives.
- Demonstrates a high level of clinical care competency specific to assessment skills, problem solving, critical thinking and solutions-based approach.
- Utilizes strong organizational skills and the abilities to prioritize based on importance and urgency to address client needs and follow through in a timely manner.
- Participates and supports pervasive company culture of “every team member sells” by developing and nurturing relationships with healthcare providers and community partners that may be involved in the client’s care.
- Demonstrates a holistic and client-centered approach when delivering services to clients.
- Devises initial plan of care assessment and periodic client assessments by identifying clients’ physical, behavioral health and social determinants strengths, barriers, and clinical needs.
- Advocates for clients at home, in the hospital or other facility, and with medical appointments to ensure highest quality and continuity of care.
- Demonstrates cultural competence, to understand and value clients’ unique perspectives.
