Position Description: The Clinical Pharmacist Specialist in Ambulatory Care is a challenging, diverse, and dynamic position with opportunities and responsibilities in ambulatory care pharmacotherapy management, teaching, mentoring, and research. The Pharmacotherapy Team consists of highly skilled Clinical Pharmacist Specialists each having completed at least one year of post-graduate training and three PGY1 and PGY2 Pharmacy Residents who strive to provide high caliber and comprehensive pharmaceutical care services.
The responsibilities of this position are to provide collaborative drug therapy management (CDTM) services in our outpatient clinics. You will collaborate with physicians under established CPAs to manage patient medication therapy, initiate, modify, discontinue medications, and administer vaccinations. Documenting progress notes and ordering lab tests for medication monitoring are also key aspects. Your expertise in disease state management, particularly chronic conditions like anticoagulation, diabetes, hypertension, hyperlipidemia, and tobacco use disorder, will be crucial in optimizing patients' medication regimens and providing medication adherence counseling. Additionally, you may have the opportunity to deliver lectures to medical/nursing staff, participate in key hospital-wide committees, and serve as a liaison between pharmacy and medical/nursing teams.
A responsibility of this position is to serve as a faculty member, role model, mentor, and preceptor for our Pharmacy Residency Programs in Pharmacy (PGY1) and Internal Medicine (PGY2). There will be a non-compensatory faculty appointment as a Clinical Assistant/Associate Professor of Pharmacy Practice with Touro College of Pharmacy. Opportunities to precept Pharm.D. students also exist. This position offers an excellent opportunity to fulfill leadership, learning, teaching, research, and publishing endeavors. Required or Desired Credentials or Experience: The applicant should have excellent oral and written communication skills. Applicants should possess a Pharm.D. degree, eligible for licensure in New York State plus certified immunizer and CDTM certification eligible. A PGY1 Pharmacy Residency is required, and a PGY2 Pharmacy Residency is required, preferably in Ambulatory Care.
Description of Institution/Organization: Wyckoff Heights Medical Center is a 300-bed teaching, medical and pharmacy university-affiliated, non-profit, community hospital located in an ethnically diverse neighborhood in Brooklyn, NY. The medical center has two 50-bed medical/surgical units, a 50-bed cardiac care telemetry unit, an 8-bed intermediate care unit, a 16-bed intensive care unit, an adult and pediatric emergency department, obstetrics, pediatrics, surgery services, and an ambulatory care center.
