Qualifications:
§ Current Hospice Aide certification (in good standing) in the state of Arizona
§ Works well within a team environment to collaborate daily with interdisciplinary team.
§ Can work independently and on your own on a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
§ Time management skills to manage visits, documentation, and meetings.
§ Ability to learn electronic medical records system.
§ Must have reliable transportation.
§ Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered.
§ Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.
§ Strives for independence and sound critical decision-making regarding the delivery of patient care in an effective manner without compromising quality.
