The LPC will serve as an Integrated Mental Health Care Provider by working with an elite team of healthcare providers and other professionals to provide therapy using brief interventions based on evidence-based practices. They will help individuals with behavior modifications, processing, coping skills, and other therapeutic interventions to improve their mental and physical health and overall wellbeing within a primary care setting. This person will work closely with our providers and other primary care staff to establish and maintain plans of care. LPC, LMSW, or LMHC are accepted.

Responsibilities:

Provides screening and symptom assessments of patients to determine appropriate modalities and intervention for use in a therapeutic setting

Works closely with providers and primary care staff when counseling patients, both individually or with family members and caregivers to ensure all members of the patient’s care team are properly informed of the patient’s state

Accurately maintains confidentiality of records relating to clients’ treatment.

Guides patients, family members and caregivers in the development of skills and strategies to successfully overcome barriers and trauma or cope with other emotional stresses.

Assists with navigating community referrals for patients appropriate for a different level of care

Coordinates mental health care with specialized, integrated care team, including supervising physician, provider, care manager, and other support staff

Completes thorough documentation about patients through interviews, observations, assessments, and other interventions in the company’s EMR

Evaluates clients’ physical or mental condition based on review of client information.

Develops and implements treatment plans based on clinical experience and knowledge.

Acts as a client advocate to coordinate required services or to resolve emergency problems in crisis situations.

Serves as a subject matter expert to the entire care team regarding behavioral health topics and trends
Required Qualifications:
-
Licensed in LPC, LMSW, CADC or LMHC

A Master’s degree in psychology, counseling or a related field required.

Appropriate state credentials and licensing required.

Experience working with all ages from adolescent and geriatric highly preferred
-
Experience working in a primary care setting preferred

Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.

Experience using electronic health records for documentation
-
Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance.

Knowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, and cultures and their history and origins.

Knowledge of principles and processes for providing customer and personal services.
Physical Requirements:
Must be able to lift to 40 pounds at times.
