Siskin Hospital for Rehabilitation is seeking a Nurse Liaison to add to our team. Are you a great communicator that loves to work with patients and their families? You'll be -
- Responsible for pre-screening potential patients and preparing documentation to determine, in conjunction with the medical staff, appropriateness of admission.
- Coordinating patient admissions when approved, including the gathering of all pertinent information regarding the admission.
- Initiating contact with referred patients and families and maintaining frequent communication with patient and patient’s family.
- Providing opportunities for patient/family to be able to make an informed decision to select SHPR.
- Providing medical information when requested by case managers or for purposes of precertification.
- Performing routine public relations activity promoting optimum communication between Admissions Department and referral sources/guests.
- Traveling to referring facilities.
MINIMUM QUALIFICATIONS OR EQUIVALENTS (education, experience, licensure, certification, and/or specific skills) *time frame for proficiency where appropriate
Experience:
1. Experience in and/or knowledge of comprehensive rehabilitation.
2. Experience in clinically completing nursing assessment function.
3. Experience in patient/family/staff education.
Licensure/Certification:
1. RN license in Tennessee and Georgia if appropriate, Alabama licensure may be required for Liaisons functioning/covering in the outreach Nurse Liaison role.
2. Valid Class D drivers license.
3. CRRN preferred.

PHYSICAL/MENTAL REQUIREMENTS
1. Strength - Position requires ability to frequently lift light objects and some heavy with lifting briefcases.
2. Manual Dexterity - Position requires incumbent to often perform simple manipulative skills such as copying, filing, collating, stapling, and moderately difficult manipulative skills as typing and data entry.
3. Coordination - Position requires incumbent to often perform tasks, which require hand-eye coordination such as typing and data entry.
4. Mobility - Position requires incumbent to often be able to sit for prolonged periods, frequently move throughout the building, and travel independently in all types of referring facilities.
5. Visual Discrimination - Position requires incumbent to constantly see objects closely as in reading patient admission charts, medical records, and prescreen forms.
6. Hearing - Position requires incumbent to constantly be able to hear normal sounds with some degree of noise as in answering the telephone and discussing matters with patients, family members and other hospital staff.
7. Concentration - Position requires ability to constantly concentrate on fine detail with constant interruption.
8. Attention Span - Position requires ability to constantly attend to a task or function for more than 60 minutes at a time.
9. Conceptualization - Position requires ability to constantly understand and relate to theories behind several related concepts.
10. Memory - Position requires ability to constantly remember multiple tasks/assignments given to self and others over long period of time.
