Compensation Range : $40,000 - $45,000/year Commensurate with experience
Overview: The purpose of this position is to promote independent living within a supportive housing program by providing intensive case management services aimed at developing and enhancing skills of clients in various aspects of their life and assisting clients to be ready for permanent independent housing upon graduation from our program.
Responsibilities:
- Manages and maintains a caseload of up to 30 formerly homeless young adults and their children.
- Maintains program in contractual compliance by completing individualized treatment plans, assessments, progress notes and other required assessment tools on time and keeps all information confidential.
- Adheres to the provision of service deliveries required in the contracts. Submits required reports on time.
- Conducts housing meetings among clients who share an apartment.
- Conducts group activities centering on themes related to young adults.
- Keeps regular contact with clients including required monthly home visit.
- Maintains agency standards and contractual mandates by following policies and procedures.
- Maintains orderly and organized charts.
- Provides accompaniments to client.
- Provides crisis intervention and mediations as necessary.
- Provides backup to other case managers when necessary.
- Provides care coordination to health, mental health, and substance abuse services by utilizing internal and external resources in order to meet the various needs of clients who were chronically homeless.
- Coordinates multiple enrollments of clients in other Bailey House programs.
- Obtains updated medical and mental health documentations for each client.
- Assists clients with budgeting, shopping, counseling regarding rent arrears, providing ADL training, and providing orientation to new clients with a program structure.
- Works collaboratively with the Vocational and Educational Specialist to assist clients obtain employment and increase income.
- Attends weekly supervision, team meetings, and trainings. The incumbent is expected to come to supervision prepared with an agenda to discuss casework challenges and successes.
- Travels via the use of mass transit with clients or by themselves in the service of clients.
- Maintains good communication skills to advocate on behalf of clients.
- Contributes to team effort by accomplishing related results as needed.
- Participates in the planning and execution of client activities as determined by program administrators.
- Maintains agency operations and workflow.
- Follows agency policies and procedures.
- Maintains professional and technical knowledge.
- Understands and complies with agency guidelines in the personnel handbook. Performs other responsibilities and duties as assigned.
Job Requirements:
- Prefer a BSW or related degree with adherence to ethical standards of the social work profession.
- Experience in working with young adults and children.
- Experience in working with people with living with HIV/AIDS and/or other chronic conditions.
- At least three years (3) of case management or related experience in housing program
- Experience working with children. Preferably infant to toddlers.
- Must possess excellent organizational and time management skills
- Must possess excellent interpersonal skills and be able to work utilizing a team approach
- Ability to make sound assessments of clients, their needs and develop treatment plans
- Knowledge of homeless services in the New York City Metro area
- Bilingual (English- Spanish) is preferred
- Must possess ability to set boundaries and limits with client population
- Strong analytical and problem-solving skills
