If our mission resonates with you, we are looking for three Full-Time Case Managers to join our highly successful, dynamic, and growing team. They will report to the Director of Social Services and work in the Residential Program.
The Case Manager will be a member of the treatment team and provide services to women working toward recovery from substance abuse and co-occurring mental health disorders. They will advocate for and provide instruction and support around the Client's bio-psychosocial needs in a manner that reflects Libertae's mission.

REQUIREMENTS:
- Bachelor's Degree, in a human services field
- Strong organizational and time management skills
- Crisis intervention and assessment skills
- Working knowledge of addiction and 12-step programs
- Competence in leading topic education groups
- Valid driver's license
- Excellent written and verbal communication skills
- Strong computer skills, experience with electronic records (EMR/EHR)
- Ability to secure Act 33 and 34 clearances
- Willingness to work flexible hours, to accommodate the Client's needs
Essential Duties and Responsibilities:
- Conduct case management assessment and develop individualized service plans, update as required
- Facilitate case management topic group and individual sessions
- Complete paperwork requirements in a timely manner; maintain quality, up-to-date clinical records in an electronic record system
- Assist residents in navigating: vocational, educational, employment, housing, financial, medical, and legal systems and interface with area social services agencies
- Assist with transporting residents using company vehicle if there is a need
- Assist residents in obtaining photo identification, birth certificate, social security cards, and past medical and school records
- Assist mothers with the needs of their children, provide parenting support
- Comply with all internal and external policies, procedures, regulations and standards; including Libertae policies and procedures, DDAP requirements, state licensing standards, county monitoring requirements and managed care regulations
