Job Title: Contracted Registered Nurse-CCSP
Reports to: Chief Administrative Officer
Job Purpose: To conduct assessments initially and annually to assist in identifying each individual’s needs and desires for service delivery to prevent the need for entering a nursing home.
Main Duties and Responsibilities
- Conduct face-to-face initial and annual comprehensive assessments for entry and continuation of EDWP services
- Evaluate the clients’ medical and health status, including functional ability and social, emotional, and financial factors related to his/her illness
- Conduct Supervisory visits for clients and re-evaluating their care needs
- Identify potential services needed to restore or preserve client health
- Obtaining and inputting client medications to determine primary, secondary, and tertiary diagnoses for creation of Level of Care
- Creation of the clients’ individualized care plans including all services being requested
- Complete Financial Worksheets with clients during initial and annual assessments to determine financial eligibility and discuss potential estimated cost share for services
- Signature on all approved Level of Cares
Required Knowledge, Skills, and Abilities
- Great communication and documentation
- Valid Georgia Driver’s License
- Maintenance of Georgia RN license
- Administrative skills: saving, uploading, faxing, scanning, and editing documents
- Clean criminal background history and child abuse clearance
- Fingerprinting
Preferred Minimum Training and Experience
- Experience assessing elderly and disabled clients preferred
- Nursing Home experience preferred
- 2+ years’ experience in providing care to elderly individuals and/or those with physical disabilities

- Residency in Atlanta area preferred as all of our clients are in the Atlanta region
- Must be willing to travel to the 10 counties in the Atlanta region: Cherokee, Clayton, Cobb, Dekalb, Douglas, Fayette, Fulton, Gwinnett, Henry, and Rockdale
Work Location: from the comfort of your home
Compensation:
· $100 per Initial Assessment of client in home
· $75 per Annual Assessment of client in home
