CLUSTER Community Services is a 501©3, non-profit social service agency. CLUSTER’s four core programs are Mental Health Residential Services, Westchester and Rockland Mediation Centers, Youth & Family Programs, and the Housing Resource Center.
CLUSTER Community Services, Inc., established in 1975, is a non-profit, multi-service, community based 501(c) (3) organization that provides services to the Yonkers community and throughout Westchester and Rockland County. CLUSTER’s mission is “To provide services and programs that assist residents in Yonkers and Westchester and Rockland County to build a better future for themselves and their communities. The department the posting relates to.
Our Mental Health Department provides critical support for some of our community's most vulnerable individuals. Ranging from congregate care facilities to supported independent community living, our programs mirror the intervention needs of our mental health clients.
Residential- support clients who are in the beginning stages of their journey towards independent living, through 24/7 direct care, counseling, and guidance. We seek individuals fully invested in providing “unconditional support” to our clients as they persevere through challenges and achieve a greater degree of self-sufficiency.
Apartment Treatment- supports clients who are living independent in the community but need “hi-touch” support and guidance by a dedicated case manager. We seek individuals fully invested in helping clients develop healthy living habits and constructive daily routines, which include taking care of their physical and mental health needs. Because of the “hi-touch” nature of this program, case managers will be expected to carry a caseload of 10-12 clients.
*Case managers are responsible for providing adequate care for all clients who reside in the facility.
Who we are seeking.
Case Managers for our Congregate Residence or Apartment Treatment Program
Our ideal candidate will have experience in and be able to:
- Provide identified and necessary support and service to residential clients to help maintain health, safety, and independence. Provision of some services may be required through the use of individual or group interventions or by referral to appropriate community resources.
- Maintains all required written records, specifically including but not limited to individual case records, medication, shift logs and communication books.
- Develops and implements individual rehabilitative service plans with residents.
- Follows all programmatic policies and procedures related to Administrative, Emergency and Direct Service practices.
- Follows all applicable laws and regulations including but not limited to those regarding provision of Medicaid Restorative Services, Health Insurance Portability and Accountability Act (HIPAA), OSHA.
- Maintains appropriate level of training including first aid and other items deemed necessary. Attends and participates in required meetings.
- Oversees and participates in maintenance of physical environment (House or Apartment).
- Maintains collaborative contacts with other individuals and agencies providing services to residents to ensure consistency in treatment planning and service provision.
- Performs other, related duties as necessary and/or assigned.

Bachelor's degree in a social science; preferred. No experience needed but preferred.
Associates degree in a social science, with at least 3 years of comparable work experience.
High School Diploma, with at least 5 years of comparable work experience.
