```Duties```
- Provide direct patient care in a dialysis setting, ensuring the highest standards of care and adherence to protocols.
- Assess patients' condition, monitor vital signs, and administer medications as prescribed.
- Perform dialysis procedures, including hemodialysis and peritoneal dialysis, following established protocols and guidelines.
- Monitor patients during dialysis treatment, making adjustments as necessary to ensure optimal therapy.
- Educate patients and their families on dialysis procedures, self-care techniques, and lifestyle modifications.
- Collaborate with interdisciplinary healthcare team members to develop and implement individualized care plans.
- Document patient assessments, interventions, and outcomes accurately and timely in electronic medical records (EMR) systems.
- Adhere to infection control protocols to prevent the spread of infectious diseases.
```Experience```
- Minimum of 3 years of experience as a Registered Nurse in a dialysis setting.
- Experience in chronic renal care or other related specialties is preferred.
- Proficiency in venipuncture and other phlebotomy techniques.
- Experience in medication administration and management.
- Minimum 1 year of Peritoneal Dialysis experience
Qualifications:
- Valid Registered Nurse (RN) license in Texas.
- BLS certification required; ACLS certification preferred.
- Strong knowledge of peritoneal dialysis procedures, equipment, and protocols; required PD training and/or certifications/experience hours a must.
- Familiarity with EMR systems such as Cerner or similar platforms.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment with a diverse patient population.
Ability to travel to and from the work site and to patient's homes as needed.
