The Role:
As a Home Health Aide at Lifeworks, you'll be based in one of our various beautiful homes in a suburban neighborhood. You will provide assistance and coverage for staff in direct care, you will support the people we serve to be empowered in their communities, and will have the ability to make a real difference in the lives of people with intellectual and developmental disabilities.

What We’re Looking For:
We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, a strong belief in enhancing the lives of those we serve and strong teamwork skills to join us as a Home Health Aide in our Residential programs. Are you someone who pays attention to the details, stays organized in a sometimes fast-paced environment, and can exercise good judgment in challenging situations? What You’ll Do:
· Collaborate with an exceptional team to plan and engage in the advocacy and support of our individuals.
· Promote optimal health for individuals through routine, preventive, and emergency health care.
· Provide support through case management, mobility and transfer assistance, incontinence care, meal preparation, home organization and maintenance, life skills coaching, and transportation needs.
· Help individuals lead more independent and secure lives in their communities.
· Other duties and/or assignments at our different locations based on the needs of the team and the individuals you serve.
RESPONSIBILITIES:
- Provide mobility and transfer assistance, which may include mechanical lift devices.
- Prepare meals and snacks. May include grocery shopping.
- Perform a variety of housekeeping activities including, but not limited to: sweeping, mopping, laundry, and linen changes.
- Assist with incontinence care.
- Assist with transportation needs.
- Attends regular staff meetings as necessary.
- Other duties and/or assignments to work/program locations based on the needs of the company.

Education & Experience
- High School diploma or general education degree (GED) required; related experience preferred.
- Personal vehicle
- Valid driver’s license (in state of residency) required.
- All candidates must be able to successfully pass a CORI, reference, multi-state background, and driving record check.
