Responsibilities:
A. Monitor and track all wound assessments, staging, and protocols as ordered by both attending and consulting physicians under the direction of the ADON/Unit Coordinator.
B. Review and assist in care plan initiatives developed by the interdisciplinary team as it relates to wound care issues.
C. Review wound care documentation for accuracy and provides training, if necessary, in conjunction with the DON, ADON.
D. Provide supervision for Nursing Department and covers staff call-offs as necessary.
E. Assist with coordination of admissions to facility including nursing assessments, development and initiating of plan of care.
F. Assist with discharge planning to ensure a safe and appropriate discharge including reviewing discharge instructions, home medication regime, follow up appointments and any additional needs with resident and/or responsible party.
G. Review and ensure appropriate documentation of resident assessments and conditions.
H. Assist DON/ADON/Unit Coordinator in coordinating and supervising the nursing service department to ensure the best possible functioning of the facility.
I. Assess unit needs, conditions and make changes essential to delivery of appropriate resident care services.
J. Respond to complaints about resident care and manages through appropriate resident care services.
K. Serve as resource to staff and educates staff on nursing clinical issues.
L. Maintain data and reports as requested.
M. Perform other related duties as required by their manager.
Infection Control:
A. Conduct an Annual Infection Control Inservice appropriately based on deficiencies in audits, resident/employee statistics and trends that may have developed.
B. Review employee sick slips in conjunction with Inservice Coordinator.
C. Review the nursing progress notes, house report daily and tabulate SNF resident infection statistics monthly.
D. Investigate and act on trends that are recognized during surveillance of the SNF nursing units, SNF resident statistics or employees. Initiate steps to decrease or change these trends as identified.
E. Make reports to New York State Deparffnent of Health on communicable diseases and/or exceeded norms of an individual kinds of infection after consulting with Director of Nursing.
F. Review and record appropriate laboratory reports. Action should be taken on issues that become evident through this surveillance.
G. Act as a resource person for infection control issues, researching, teaching, supervising and providing guidance to staff.
H. Maintain and update all infection control policies and procedures, including Exposure Control Manual at least annually.

Pay: $85,000.00 - $95,000.00 per year
