RN- Assisted Living Service Coordinator

Overview:
We are seeking a highly organized and detail-oriented Service Coordinator to join our team. As a Service Coordinator, you will be responsible for coordinating and managing various administrative tasks to ensure the smooth operation of our services. This is a crucial role that requires excellent communication skills, the ability to multitask, and a strong attention to detail.
Responsibilities:
- Provides referral services for residents needing such services; coordinates and develops services through community resources, which may include case management services, counseling, preventive health and educational programs and legal or other advocacy services.
- Manage resident needs, identifies and monitors the physical and cognitive health using basic assessment skills and functional assessment tool; observes stability or difference in behavior patterns and provide exceptional customer service.
- Maintain accurate and up-to-date resident records.
- Collaborate with healthcare professionals to ensure proper care coordination.
- Assist in the development and implementation of care plans.
- Monitor service delivery and address any issues or concerns.
- Conduct assessments to determine resident needs and working with the admissions Coordinator to screen eligibility candidates.
- Provide support to residents and their families throughout their stay at our facility.
- Maintain confidentiality of resident information at all times
- Participates in the development of the organization’s overall plans and programs in relation to Resident Services. Assists in planning and contributes to the philosophy and goals of the entire organization.
Qualifications:
- Active Vermont RN License
- Previous experience in a similar role preferred.
- Strong organizational skills with the ability to prioritize tasks effectively.
- Excellent communication and interpersonal skills
- Proficient in using computer software and databases.
- Knowledge of medical terminology and procedures is an asset.
- Familiarity with electronic medical records systems (e.g., Cerner, MatrixCare) is a plus.
Skills:
- Shall be a graduate of an approved school of nursing and hold a license as a registered nurse in the State of Vermont.
- (S)he shall also understand elderly persons and their psycho-physiological functioning.
- Shall have at least two years' experience as a Charge nurse or Social Services Coordinator.
- Prefer experience as advocate in similar setting.
- Proven effectiveness as a facilitator.
- Awareness of community activities appropriate for current residents.
- Ability to identify solutions/resources to empower residents’ self-sufficiency.
- Ability to communicate in effective manner via written or oral skills.
- Computer skills – experience with Word, Excel preferred.
- Must possess current active license with the State of Vermont
- Experience in gerontology preferred.
- Experience as a supervisor in a hospital, long term care facility or other related health care facility.
- Must possess professional clinical skills.
- Must be willing to work with other departments in a harmonious manner.
- Must have reliable means of getting to work.
- Must have the availability of being reached by phone.
