Job Summary:
We are seeking a compassionate and dedicated Hospice Nurse to join our team. The Hospice Nurse will provide end-of-life care and support to terminally ill patients and their families. This role requires a high level of empathy, clinical expertise, and the ability to deliver personalized care.
Duties:
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient vital signs and symptoms
- Provide emotional support to patients and families
- Collaborate with interdisciplinary healthcare teams
- Educate patients and families on end-of-life care options
- Ensure comfort and dignity for patients in their final days
Requirements:
- Valid RN license in [State]
- Previous experience in hospice or palliative care preferred
- Knowledge of sterile processing procedures
- Familiarity with occupational health standards
- Experience in hospice home care
- Understanding of internal medicine practices
- Proficiency in ICD-10 coding
- Ability to assess and manage complex patient needs
- Background in behavioral health is a plus
