Job Title: Case Manager (Single Site & Scattered Site)

Educational Requirements: High School diploma /GED plus 4 years relevant experience; or College (BA/BS) or Master’s Degree.
Qualifications and Experience: Experience working in mental health, homelessness, supportive housing, crisis intervention and/or managing trauma triggered behavior.
Scope of Position:
Case Managers are responsible for providing supportive services to formerly homeless adults diagnosed with serious mental illness and/or substance use disorders, who are living in independent apartments either in a single site location or scattered in the community.
Essential Position Functions:
1. Provide case management services to clients, including assistance with obtaining/recertifying for public benefits; establishing/maintaining links to primary and behavioral health care; supporting activities of daily living skills; utilizing community services; shopping; etc. These services are primarily provided in client apartments and in community settings.
2. Assist in identifying client strengths, needs, abilities, and goals and create related Support Plans in electronic client records system.
3. Conduct apartment inspections on multiple floors, often requiring the use of stairs.
4. Provide escorts to clients throughout the community utilizing public transportation.
5. Timely completion of required documentation for every client contact.
6. Co-facilitate groups in areas including but not limited to wellness, housing stability, employment and education, relationships, and healthy decision making.
7. Assist in preparing program for audits.
8. Provide crisis intervention, as necessary, utilizing de-escalation techniques.
9. Collaborate with other internal departments, outside agencies and treatment providers.
10. Ability to work with clients, families, and staff in a caring and respectful manner with consideration for cultural differences.
11. Attend staff meetings, supervision and trainingS as required.
12. Any other duties as may be assigned.
