COMMUNITY INTERVENTION SPECIALIST Requirements:
- Master’s Degree from an accredited college or university, specializing in psychology, social work, or guidance and counseling
- Current licensure/certification issued by the State of Michigan
- Shall have provided direct treatment services to adults in a behavioral health inpatient or outpatient setting for not less than 18 months
- Demonstrate basic knowledge of diagnostics, psychopharmacology, and supportive treatment approaches as applied to a severely mentally ill (SMI) adult population
- Knowledge of the identification and treatment of co-occurring mental health and substance abuse disorders
- Possess a chauffer’s license (or acquire one during the onboarding process)
COMMUNITY INTERVENTION SPECIALIST Responsibilities (not all inclusive):
Intake
- Complete assigned new intake comprehensive bio-psychosocial assessment within seven days.
- Formulate preliminary DSM five-axis diagnosis.
- Identify issues that may exist regarding safety (personal and environmental), natural supports, and community involvement.
- Identify referrals needed for comprehensive treatment, including medical, nutrition, substance abuse, STD/HIV, education, and vocational training.
- Obtain releases of information for coordination of care.
- Educate patient and patient’s parent/guardian with respect to admission information packet.
- Demonstrate knowledge of patient payor source.
- Complete Community Mental Health/CareLinkfunding Eligibility Assessment.
- Schedule appointment for psychiatric evaluation within appropriate timeframe.
Treatment Planning and Progress Reviews
- Complete an individualized Treatment Plan for each patient.
- Utilize the patient’s Person Centered Questionnaire in development of treatment plan and support person participation.
- Treatment Plan problem areas, goals, and objectives address the patient’s DSM five-axis diagnosis and areas identified on the bio-psychosocial assessment.
- Demonstrate use of natural supports in Treatment Plan.
- Complete Treatment Plan Reviews (TPR) minimally after the first 90 days of treatment and 180 days thereafter.
- Update Treatment Plan via a TPR following a change in clinical status to change in diagnosis, hospitalization, or utilization of crisis services.
- Utilize HPI corporate, continuous Treatment Plan process when receiving or facilitating an internal patient transfer.
Discharge Process
- Document on Treatment Plan criteria and anticipated date for planned discharge.
- Discharge patients with no contact for 45 days.
- Complete Discharge Summary within 21 days of discharge date.
- Complete Patient Discharge Demographics form within 21 days of discharge.
- Complete a Continuing Care Plan for each patient discharge.
Ongoing
- Coordination of appointments with psychiatrist and other primary therapists.
- Provide linking and coordination of community-based services.
- Transport patients as needed to community-based supportive, adjunctive services.
- Provide individual supportive, solution-focused psychotherapy not less than once every 30 days to all patients on caseload.
- Provide group therapy, as assigned, demonstrating utilization of supportive and/or psycho-educational approach, based on assignment.
- Document all patient contacts and/or contacts related to case management services.
- Initiate contact, by phone or in writing, to non-compliant patients within 24 hours of a missed appointment.
- Communicate with hospital staff and/or hospital liaison in the event of an inpatient admission.
- Meet with patient in hospital within two (2) working days of notification of hospitalization.
- Ensure a minimum of three contacts with post-hospitalized patients within ten days of hospital discharge.
- Complete a progress note for all treatment services.
- Assist patients and/or other clinical staff with various case management tasks, including but not limited to, medication compliance, referrals, and linkage to resources.
- Provide24-hour crisis intervention services to patients, as necessary.
Pay is based upon years of related experience for limited license applicants. Hegira Health supervises hours needed for full licensure at no cost. Additional compensation for full license.
