HOTBHN RRH Housing Case Manager works to provide supportive and case management services to RRH consumers, while empowering consumers to become self-sufficient in order to maintain housing status after program completion. Providing life skills training, problem solving skills, and connecting with community resources to enhance individuals housing outcomes.
EXAMPLES OF WORK PERFORMED::
- Responsible for providing supportive case management services, for all RRH consumers.
- Provides life skills training to RRH participants, such as comparison shopping, budgeting, money management, independent living skills, etc., to enhance individual housing outcomes.
- Provides problem-solving skills, such as conflict between neighbors or landlord.
- Assist with creating budgets to maintain bills and housing needs.
- Coordinates with community landlords in the effort to build community wide TBRA options for consumers.
- Completes annual housing inspections to make sure safety and health living standards are in compliance with HUD guidelines.
- Coordinate with landlords to resolve potential conflicts.
- Acts as a liaison to the Heart of Texas Homeless Coalition’s Heart to Home Coordinated Entry system while also performing other necessary Coordinated Entry functions.
- Complete all documentation in an accurate and timely manner to maintain HOTBHN compliance.
- Connect consumers with community resources.
- Performs other related duties as required.
KNOWLEDGE, SKILLS AND ABILITIES: :
- The position requires a working knowledge of mental illness its impact on homelessness.
- This position requires the ability to work with a wide variety of community partners as well as disabled individuals and individuals with mental illnesses. You will need to be able to form positive relationships with landlords and other community partners to advance.
- This position will require a great deal of independence and ability to problem solve. The successful transition from homelessness to housed will require the TBRA CM to be flexible, creative with resources, and think outside of the box.
- This position will require excellent computer skills.
- This position will require excellent time management and organizational skills.
- Excellent verbal communication with consumers, staff, and community providers
- Stooping, bending, sitting, standing, keyboarding for prolonged periods, and lifting 15 lbs.
- Ability to be CPR and PMAB certified
- Valid driver's license, liability insurance, and reliable transportation required. 30% local travel
Education and Experience
- Bachelor's or Master's in social work, psychology or related field with ability to obtain Qualified Mental Health Professional credentialing
- 1 year of case management and knowledge of community resources preferred: but not required
Hours: M-F 8AM-5PM with other as needed
Salary:
Bachelor's-$45,000-$49,500 Annually
