WHAT WOULD I BE DOING AT THIS JOB?
The Case Manager is a member of the Behavioral Health Department working closely with the Medical, Dental and Behavioral Health clinicians to support patients who would benefit from additional assistance.
Responsibilities
- Use clinical tools, data, and other information to complete assessments of patient needs create individualized and comprehensive case management and treatment plans
- Clearly explain the reason for prescribed treatments and treatment plans to patients.
- Communicate with patients, physicians, and specialists to secure successful outcomes
- Document medical information in the EHR
- Follow case and quality management processes in compliance with regulatory guidelines and company procedures
- Participate in Quality Improvement Activities
Qualifications for Case Manager
- Associate's degree, or bachelor's degree preferred
- Strong written and verbal communication skills with patients, physicians, and other stakeholders
- Demonstrated ability to assess and respond to patient and family needs in a timely manner
- General knowledge of case management practices, including familiarity with relevant products and services
- Basic computer skills, including working knowledge of the Microsoft Office suite
- Experience in establishing professional and collaborative relationships with patients and doctors
- Experience in behavioral health, counseling, or case management required

Pay: $30.00 - $35.00 per hour
Expected hours: 40 per week
