Lead RN Care Coordinator – Registered Nurse

Schedule:
- Full time, Monday-Friday, 8 am - 5 pm
- No weekends, nights or on-call!
Compensation: $60-$65/hr, dependent on experience, plus a 10% performance bonus

Lead RN Care Coordinator Responsibilities:
- Interact with an interdisciplinary team to help coordinate in-home services and transitional care services
- Communicate discharge information to clinical and administrative staff inside and outside of the Hospital
- Provide telephonic care management services
- Help patients recover from acute illnesses
- Educate patients, caregivers and family members on in-home best practices
- Help develop individualized care plans and ensure appropriate follow up through the care continuum
Qualifications:
- Associate’s Degree in Nursing (RN)
- Bachelor’s Degree in Nursing (BSN), preferred
- An active Registered Nurse license in the state of New York (RN)
- 2+ years of hospital/acute care experience (Med/Surg, ICU, Emergency Medicine, Telemetry, etc.), required
- 1+ year of case management / care coordination experience, preferred
