Job Description: Works as part of a team to ensure that clients are treated with dignity and respect and that they have a safe, healthy living experience in the Shelter. Provides case management services - assessment, referral, coordination of services, to ensure that residents/clients/consumers move as rapidly as possible toward independent living and greater self-sufficiency.
Duties and Responsibilities:
* Complete a Service Plan that outlines clear goals, time frames, and responsibilities that address issues that impair and/or inhibit the resident/client/consumer from achieving self-sufficiency within 30 days of resident/client/consumer entrance into the Shelter.
* Meet with resident/client bi-weekly and monitor resident/client adjustment to the Shelter and compliance with the Service Plan, provide information and referrals to needed services, collaborate actively with other members of the Service Team, assist the resident/client to develop a housing plan.
* Utilize DSS Cares Dashboard to identify client/resident with multi-agency involvement and verify accuracy of information with resident/client.
* Utilize the Homeless Management Information System (HMIS) to record and store client-level information on the characteristics and service needs of residents/clients.
* Work alternative hours, evenings, and weekends as needed and required.
* Provide crisis intervention and brief supportive counseling to resident/client either separately or in coordination with other partner support services.
* Work collaboratively with Residential Advisors/Shift Supervisors, Housing Coordinator, and all other ancillary services particularly around issues of housekeeping and curfew, budgeting, savings and fees, children's school attendance, progress, and parenting.
* Participate in regular Administrative Review Meetings and scheduled Progress Review Meetings. * Perform other case management tasks as directed by the Case Management Supervisor, Associate Program Director/Program Director.
*Able to climb flights of stairs and lift at least 25lbs.
* Perform all other duties as assigned.
Qualifications: Minimum BA Degree required (Social Work preferred); Bilingual preferred; current driver's license; able to lift and carry up to 30 Ibs; 4+ years experience providing direct services; experience in behavioral health, child welfare, subsidized housing, and/or vocational assessment is highly desirable; teamwork skills required; must have great computer skills. E-mail, Microsoft Word, and Outlook friendly, able to type 40-50 words per minute, able to navigate Internet; able to pass State Child Care Clearance and Criminal Background checks is a must.
**Experience in D&A and Mental Health or Transitional Housing is a plus, Bilingually preferred.**
