NYIA (New York Independent Assessor Program)
Seeking board certified Nurse Practitioners FNP, ANP, AGNP, - to support the New York statewide NYIA Independent Practitioner Panel that issues physician orders for individuals who have undergone a Community Health Assessment and are eligible for community-based long-term care services and supports. The NP will follow up with individuals who have received an assessment.
Western New York, Albany Capital region, Central NY, Southern Tier, North Country, Hudson Valley.
- $115.00 per hour – community health in-home assessments
- Must be board certified and carry personal medical malpractice.
- Must have an active NYS Medicaid ID number.
- Must be conflict free; cannot have a Conflict of Interest (COI) - including working for an MLTC, LHCSA, Certified Home Health Agency, Insurance or ISNP
- Must have at least 2 years (3600 hours) of prior experience in any setting to qualify.
- Schedules available Monday – Friday 8am – 7pm; Saturdays and Sundays 10-6.
- Laptops are provided by the client.
- Training is virtual classroom, instructor-led 15 hours (paid).
- 1099 role
JOB RESPONSIBILITIES
- Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports.
- The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, in order to assist the Physician in the completion of a Physician’s order.
- Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments.
- Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes.
- Work collaboratively to monitor and address clinical quality and gaps in care
