Premier Home Health Care Services, Inc. is a leading multi-state established home care agency. We are currently looking for a CDPAP Liaison in New York City.
The role of the CDPAP Liaison is to broaden the company’s presence within the community, educating the population in which we serve, and processing the initial enrollment paperwork for both Consumers and Personal Assistants. Responsibilities include:
- Increase Consumer and Personal Assistant censuses in the CDPAP to achieve revenue goals, and to educate the community on the services, benefits, and mission statement of Premier.
- Creating an expedited enrollment process, participating in community outreach efforts, social media engagement, building successful rapports with referral sources, and through tracking trends of conversions versus non-conversions.
- Assists in facilitating obtainment of Consumer’s doctor’s orders.
- Verifies potential Consumer Medicaid Eligibility.
- Trains all Personal Assistants on electronic visit verification and scheduling
Qualifications:
- Minimum of a Bachelor Degree in Business or Healthcare
- Two to three years of healthcare business development experience in a comparably sized company.
- Excellent verbal and written communication skills
- Bilingual English ~ Spanish
- Proficient in the use of computerized systems.
- Works well under pressure
