Program Summary
The Family Practice & Counseling Network (FPCN) provides comprehensive primary care, dental and behavioral health services. The Network consists of three (3) federally qualified health centers (FQHC) in Philadelphia:
- Abbottsford Falls Family Practice & Counseling (Northwest Philly);
- Stephen & Sandra Sheller 11th Street Family Health Services of Drexel University (North Philly); and
- Health Annex (Southwest).
Today the FPCN serves more than 20,000 patients annually and is a NCQA certified Patient-Centered Medical Home and recognized by HRSA as a National Quality Leader for Behavioral Health.
Position Summary
The Care Coordinator is responsible for the completion of clinical and administrative tasks in primary care to ensure that patients receive health care that is competent, caring, and meets with a high degree of satisfaction. The Care Coordinator is a key linkage person in our Care Management Program, who helps patients navigate between services and resources at the site. All Care Coordinators are expected to uphold the network’s mission of trauma informed care.
Responsibilities/Duties:
- Schedules and confirms appointments.
- Documents all activity into any pertinent electronic record
- Monitoring, tracking and facilitating communication round all community-based healthcare services.
- Follow-up on care gaps (labs, screenings), TOC changes with medication and prescriptions
- Assist with care coordination efforts set forth by Accountable Care Organizations (ACOs).
Required Education:
- Medical Assistant Diploma – State Certified
- HS Diploma/GED
Required Experience:
- 3 plus years of experience in a Medical Assistant Position
Required Qualifications:
- Current Medical Assistant Certification
- Current CPR Certification
- COVID Vaccinated

This currently does not include boosters.
