The Case Manager is a team member of the Open Hearts Outpatient Services department. They provide general case management services to enrolled clients and their families.
*Essential Functions (duties):*
*Direct Service duties, include, but are not limited to:*
* Provide comprehensive services to all assigned clients and their families to include assessment, diagnosis and treatment.
* Provide as needed case management, family support, behavioral coaching, life skills training, service planning and other support duties as needed.
* Act as a liaison with referral sources and other community agencies on behalf of the clients participating in Open Hearts services.
* Completing assessments annually or as clinically indicated.
* Complete, submit, and scan any and all service plan related documentation within 14 days of CFT meeting.
* Monthly contact with all families on active roster, to be documented in the clinical record.
* Facilitate and participate in Child and Family Teams as assigned.
* Update electronic schedule by close of business Friday for subsequent week.
* Direct service, face to face encountering will be a total of 55% of monthly productivity goal.
* Provide on-call coverage as part of the outpatient team rotation
* Administrative responsibilities include, but are not limited to:*
* Updated demographics every six month, or as clinically indicated.
* Report to CM Team Lead any wellness checks or pending closures, to be reported on a monthly basis.
* Complete referrals, per CFT Service Plans, within 14 days of initial request
* In collaboration with supervisor, submission of any OOH packets and HNCM referrals as clinically indicated, within 7 days of initial request.
* For bi-lingual staff- translation of all service plans must be completed within 14 days, as above mentioned timeframe.
* Mandatory attendance to all individual or group supervisions, and monthly team and department meetings.
* Complete all required documentation and billing requirements accurately and in the assigned time frames.
* Support and comply with the 12 principles as defined in the Arizona Practice Model.
* Assist in actively assessing work area for non-compliance issues and notify supervisor in a timely manner to ensure follow up.
* Attend all mandatory annual trainings, supervisions, and department/agency meetings as assigned.

*Preferred Education and Experience:*
* Bachelor’s degree in a related field with 2 years of experience in the behavioral health field preferred, or 60 hours of coursework in behavioral health related classes with four years of experience working in the behavioral health field preferred.
*Prerequisites:*
* Must be 21 years of age.
* Possess an Arizona driver’s license and state mandated auto insurance.
* Possess or be able to obtain a Level One Fingerprint Clearance Card.
* Must be able to pass a 5-panel drug screen
