Job Summary:
Clincal Primary Care Provider
- Perform full exam encounters
- Disease management
- Acute follow ups
- Urgent encounters prn
- Transitional care management
- Coordinate with ancillary services to provide additional care (DME providers, home health providers, Medcaid case managers)
- Diagnostic review, medication management, and disease teaching of common conditions such as CHF, DMII, COPD, etc.
- Coordinates with clinical care team support staff regarding care management
Qualifications:
Must be licensed in the stat of Oklahoma, APRN or PA preferred. Must hold a current non-restricted prescriptive licesnse from OBNDD and DEA. Oversight physician will be provided, and is actively engaged in care management and case reviews. Family practice or geriatric experience is a must.

About the role:
As out clinical primary care provider, you would play a crucial role in delivering care to patients, with a focus on preventative care, health promotion, and disease management. As the clinical primary care provider, there would be responsibilities for diagnosing and treating a variety of acute and chronic medical conditions. This can involve conducting physical examinations, ordering diagnostic tests, prescribing medications, and providing on-going monitoring and follow-up care. As clinical primary care provider you would work with patients to assess their risk factors, provide counseling on healthy lifestyle choices, offer vaccinations and screenings, and create personalized prevention plans to maintain or improve their overall health. Clinical primary care providers develope long-term relationships with their patients, which helps to coordinate their healthcare needs and ensures continuity of care, this would include managing patients' medical records, review of their medical history, provide routine check-ups and refer them to specialists when necessary. As a primary care provider, you would educate patients about their medical conditions, and provide guidance on managing their health. You may also offer counseling on mental health concerns, stress management, and lifestyle modifications to help prevent disease progression. Primary care providers often collaborate with other healthcare professionals, such as nurses, pharmacists, and specialists to ensure comprehensive patient care.
