The Baltimore Medical System Anticoagulation Program Manager plays a crucial role in ensuring safe and effective medication therapy for patients receiving anticoagulation treatment. They collaborate with healthcare teams to optimize therapeutic outcomes and minimize adverse effects related to anticoagulant medications. This position requires a deep understanding of anticoagulation therapy, effective communication skills, and a commitment to patient care and safety. This position is responsible for managing all anticoagulation patients within the Baltimore Medical System. 20 hours per week.
Clinical pharmacists ensure safe, effective, and cost-efficient medication therapy is implemented for all patients in their care and monitor care appropriately. This includes preparing medications, participating in the drug distribution process if needed, monitoring drug therapy, providing drug information to patients and providers, providing clinical pharmacy services, solving medication-related problems for individual patients as well as in the medication use process, and supervising daily workflow to accomplish daily tasks allotted to the pharmacist. Clinical pharmacists participate in quality improvement projects to improve clinical care or pharmacy operations. #BMSSJ
Required Education:
- Bachelor's Degree in Pharmacy required.
- Doctorate of Pharmacy and PGY1 Ambulatory residency or equivalent experience preferred.
Required Certifications, Licenses, Registrations:
- Pharmacist license
- CPR
- Current BCPS, BCACP, or BCCP (board certification will be required post-employment for credentialing)
Preferred Certifications, Licenses, Registrations:
- CACP Certification-National Certification Board for Anticoagulation Providers (NCBAP)
Required Experience:
- Two years of clinical experience in anticoagulation care
- Two years of direct patient care experience working in medical care systems, diverse populations, and/or underserved communities
- Strong understanding of anticoagulation therapy, pharmacokinetics, and pharmacodynamics.
- Excellent communication, interpersonal, and teamwork skills.
- Ability to analyze data, make informed decisions, and prioritize patient safety.
