Job Title: Direct Support Provider/Professional
Our company is seeking an outgoing, energetic, patient, compassionate individual to provide direct care, positive direction, and assistance to individuals with intellectual and developmental disabilities. The best candidate will be someone that has excellent leadership skills, comfortable working independently, reliable and responsible. A direct support provider will work with up to six individuals in their community, transporting them to and from recreational activities and assisting them with achieving personal goals. Emphasis will also be placed on making connections with others in the community and reaching a more independent state of living.
Some examples of activities providers will do with the individuals we serve are: working out, volunteering, hiking, bowling, playing outdoor and indoor games and sports, swimming, and doing arts and crafts.
Responsibilities:
- Help individuals establish and maintain relationships and a sense of belonging.
- Encourage community involvement and teach safety awareness.
- Help individuals develop positive and appropriate behaviors in all settings.
- Support and encourage responsible choices.
- Teach appropriate socialization skills.
- Encourage routine and consistency.
- Help individuals establish natural supports and promote self-advocacy.
- Transport individuals to and from activities in the community.
- Assisting individuals to increase their basic life skills through guidance and role modeling.
- Engaging in activities with clients and motivating them to participate in each activity (bowling, swimming, volunteering, etc.)
- Providing person centered supports- assisting each individual with reaching personal goals.
- Responsible for daily activities, welfare, progress, supervision, and learning experiences of individuals receiving services.
- Responsible for being a positive embodiment of Shine in all interactions with individuals receiving services, parents, caregivers, coworkers, and supervisor.
- Promote community awareness of the abilities of the individuals with disabilities while maintaining reasonable confidentiality.
- Expectations of continuous teamwork and professionalism not only with individuals served, but with coworkers and supervisor.
- Be willing and able to assist with activities of daily living (ADLs) including but not limited to cleaning face, assisting with clothes, toileting, changing, etc.
- Document and report incidents appropriately with the given time frame.
- Document the daily services provided, activities participated in, and progress toward goals.
- Have all paperwork completed to Shine standards and submitted in a timely manner.
- Other duties as needed
Job Requirements:
- Dependable to work scheduled hours.
- Skills: communication, adaptability, multi-task, teamwork, time-management.
- Ability to think critically and practice basic problem-solving skills.
- Provide direct assistance and support to individuals through a variety of services such as community access, personal care, and mentorship activities.
- Utilize community resources to assist the client in carrying out his or her plan in the community.
- Ensure that services provided comply with the client's service plan and goals, including the frequency of services.
- Complete the required paperwork within the given time frame.
- Provide transportation to and from community resources as indicated by the service plan and maintain a good driving record.
- Attend meetings, as necessary, usually no more than 1 per month.
- Ability to work independently.
Qualification Requirements:
- 21 or older
- Experience is preferred but not required.
- Pass Background checks and MVR
- Must have reliable transportation, a valid driver's license, and a willingness to transport clients in your vehicle (mileage reimbursement from company). Company vehicle use with larger groups.
