Enrollment Specialist - Must be Bi-Lingual
Position Summary
CareCollab provides Health Home care coordination services to high-utilization Medicaid members with chronic medical and behavioral health conditions. We’re currently seeking a dynamic Enrollment Specialist to engage and enroll eligible individuals in Care Coordination services and assist with member retention needs. The Enrollment Specialist conducts telephonic, written, and face-to-face outreach and retention services to coordinate care for Health Home Care Coordination eligible members. The Enrollment Specialist works closely with the Care Coordination team to identify, locate, engage, and enroll members eligible for Health Home services. The Enrollment Specialist assists the team by providing information about community resources that can support member wellness goals.
Essential Duties and Responsibilities
- Provide comprehensive education to potential Health Home members that highlights the benefits and promote participation in care coordination services.
- Track and document all outreach attempts and enrollment in various software portals as required.
- Complete all enrollment documentation as required in a timely fashion.
- Utilize internet databases, health record systems, and other resources to explore client eligibility and contact information.
- Track the outreach and engagement of all Lead Health Home referrals as required.
- Gather, review, and file medical, mental health & substance use reports from respective providers as needed.
- Ability to visit members in the field as needed.
- Update and maintain accurate records on prospective clients of services rendered on required software platforms.
- Actively participate in any organizational activities (meetings, events, etc.).
- Represent the agency at various venues to promote agency services.
- Responsible for maintaining and tracking the progress of reaching those members, their assignments, and their outcomes. Maintain contact with the members from enrollment until assignment to a care coordinator
- Responsible for engaging community referrals (bottom-ups) to educate them on the benefits of care coordination services and have them sign consent forms for service.
- Responsible for establishing relationships that are necessary for continued engagement and care coordination services within the community.
- Perform various clerical duties: Filing, Medicaid verification, assembling and disassembling charts, developing client contact sheets, updating client caseload, assisting the team in compiling information for the monthly report, etc.
- Conduct assigned administrative duties (mail, follow-up, etc.) and other assigned job duties.
- Mass mail outreach and engagement letters to members on the current census, as well as outreach and disengaged members.
Qualifications:
- High School diploma required.
- 1 year of job-related experience providing medical, mental health, or substance-focused services to individuals with chronic medical conditions and/or serious behavioral health conditions, preferred
- Excellent interpersonal skills with the ability to interact culturally, linguistically, and diplomatically with diverse internal and external audiences.
- Strong organizational skills to manage multiple priorities in a time-sensitive manner.
- Strong telephone personality to engage members and promote services.
- Ability to engage individuals and diffuse difficult situations.
- Health Home experience or equivalent program preferred.
- Excellent written and verbal communication skills.

Compensation:
Starting at $18 an hour Commensurate with Experience
Job Type: Full-time
Pay: $18.00 - $21.00 per hour
