**Overview:**
REPORTING RELATIONSHIPS: Direct Supervision to all Direct Care Staff
JOB SUMMARY
Provides leadership to all program employees at the site, using team-based management techniques. Performs designated coordinator functions using Personal Outcomes philosophy and standards and out based quality standards. Manages all site operations in an efficient and timely manner and provides direct care using Creating Positive Environments tenets.
**Duties:**
- Evaluates program efficiency and effectiveness by performing Designated Coordinator functions and responsibilities according to established timelines ensuring satisfactory conditions at each site.
- Assists individuals in identifying personal outcomes and individual preferences. Ensures that all services/outcomes are recorded on the agency’s documentation sheets.
- Provides leadership and supervision, directly to the direct support professionals
- Provides Direct Care as needed
- Oversees and performs administrative work, on predetermined schedules
- Maintains a professional demeanor by observing standards of conduct and integrity in the workplace, by working cooperatively with team members, and fulfilling training requirements, thereby ensuring a respectful workplace, effective team work, and qualified employees
*Description of Task Requiring Physical Demands & Frequency*
- While on outings, walks with individuals (OCCASSIONALLY)
- May have to push a wheelchair – up to 175lbs (occassionally)
- May have to lift or carry during transferring - up to 125lbs (OCCASSIONALLY)
- May have to climb stairs (occassionally)
- Reaching, and handling, may have to cover a shift until DSP comes is required (FREQUENTLY)
- Flexibility in scheduling (FREQUENTLY)
*SKILLS, KNOWLEDGE & CREDENTIALS*
1. Designated Coordinator status as defined in the consolidated rule
2. Program skills acquired by two or more years of full-time equivalency employment in a residential MR/DD setting in a coordinator/manager level position. One of the two years must be in a 24-hour program
3. Supervisory skills acquired by a minimum of 1-year experience in a supervisory position
4. Strong written and oral communication skills. Ability to develop and maintain professional relationships
5. Ability to work independently with minimal instruction
6. Ability to practice team-based management
7. Ability to balance requirements of multiple and varied duties
8. Ability to perform basic arithmetic calculations. Computer skills
9. Associate/bachelor’s degree as a Social Worker. Business Management or Pyschology (Not Required but strongly recommended)

Pay: $46,000.00 - $52,000.00 per year
