Purpose and Objectives: Provide seamless care of clients and continuity of supervision of case managers working directly with older adults age 60 and older in order to identify client’s needs and service preferences, and work with client (and family, when appropriate) to develop, implement and oversee a care plan to meet mutually agreed upon goals.

Position Qualifications:
§ Master’s Degree in Social Work (or related Master’s level degree)
§ Minimum 3 years’ experience in social service organization working directly with older adults
§ Social work supervisory experience required
§ Fluent in a second language (preferred)
§ Crisis Management Skills
§ SIFI certified, or qualified to obtain certification
Requisite Skills and Capabilities:
§ Excellent written and verbal communication skills, detail-oriented
§ Strong Computer Skills: fluency in Microsoft Word, Excel, Outlook required, Knowledge of DFTA database STARS is a plus
§ Leadership: ability to identify strengths and weaknesses in staff assigned and provide supervision, education and guidance to improve their job performance
§ Team Building: ability to create collaborative work environment where staff contribute toward common programmatic goals and support one another in their daily work

Responsibilities:
§ Perform pre-screening and initial intakes, including full assessment of needs and safety as well as assist with monitoring calls; provide information and referral accordingly
§ Carry a small caseload
§ Support Program Supervisors as directed by Program Director, including providing clinical support for high risk/high needs clients
§ Supervise Master’s and Bachelor’s level Case Managers during Program Supervisor vacancy, as assigned by Program Director
§ Conduct supervisory reviews as part of quality assurance as assigned by Program Director or Assistant Program Director
§ Assist with urgent home visits and assessments
§ Supervise social work interns, including weekly supervision and oversight of documentation
§ Supervise temp CM staff
§ Caseload coverage as assigned by Program Director
§ Authorize EISEP homecare, housekeeping services and home delivered meals as per need and eligibility requirements
§ Assist with managing PEC clients awaiting to be assigned, as directed by Program Director
§ Participate in staff meetings, supervisory meetings and NYC aging and CASC trainings
§ Maintain up-to-date knowledge of and working relationships with community resources
§ Worth with Program Director and Assistant Program Director to enhance in-service training related to case management skills and functions
§ Maintain statistical and programmatic data as required
§ Maintain familiarity with all relevant DFTA standards, including updates to standards and procedures
§ Assist with weather/emergency advisory calls and any special projects, as assigned by Program Director
§ Assist with other program activities as assigned

Supervision and Education:
§ Supervised by Program Director

Salary and Benefits:
§ $70,000-$75,000 per year
