Job Summary
Gloucester Mathews Care Clinic (GMCC) in Gloucester, Virginia is seeking an energetic and dedicated Care Coordinator, RN to help carry out our mission. The mission of GMCC is to provide comprehensive healthcare services addressing the unmet needs of our community.
Position: This position serves in a front-line role for GMCC, interacting with patients and ensuring the delivery of quality care in compliance with the standards, expectations and values of our organization. Ensures that all nursing functions within the department run smoothly and efficiently.
ESSENTIAL FUNCTIONS
· Provide guidance, support, and monitor clinical staff, manage clinical flow, and direct nursing staff in day-to-day tasks
· Coordinate patient care with physicians, clinical staff, front-end coordinator, and case manager
· Address Patient Needs or Complaints and respond to routine patient inquiries and/or telephone calls
· Create Staffing Schedules, nursing assignments, and work collaboratively with the Director of Patient Care to address staffing needs/concerns
· Ensures timely, thorough documentation of all patient interactions in accordance with medical-legal standards and Clinic policies.
· Assists Director of Patient Care in developing and implementing new and evidence-based standards of care
· Training and Orienting New Hires (core staff and volunteers) and implementing new programs
· Overseeing safety and compliance, and ensuring that organizational regulations are met
· Reviews labs, imaging, and all other incoming patient documents and makes appropriate physician notification
· Provides patient education and monitors patient compliance.
· Facilitates patient referrals and works collaboratively with the Director of Patient Care and the Referral Coordinator
· Mentors clinical team members and volunteers.
· Accurately tracks and reports data as directed.
· Inventories and maintains stock of medical and laboratory supplies and ensures exam rooms are properly equipped.
· Maintains medical equipment as appropriate.
· Actively participates in staff and committee meetings as necessary and appropriate.
· Complies with HIPAA (Health Insurance Portability and Accountability Act) and OSHA (Occupational Safety and Health Administration) training requirements and protocols.
· Other duties as assigned.
POSITION QUALIFICATIONS
Education - must be a graduate of an accredited nursing program.
Licensure - must be a licensed Registered Nurse (RN) in the Commonwealth of Virginia or Compact State .
Must have a current American Heart Association Healthcare Provider Certification.
Experience - Nursing experience in an ambulatory clinic preferred. Must be a licensed Registered Nurse (RN).
· Proficient at performing basic medical/technical procedures.
· Experience using an Electronic Medical Record (EMR) (Athena One Healthcare) preferred.

· Strong analytical, critical thinking, and conflict resolution skills.
· Great organizational and time management abilities.
· Strong office equipment and computer skills. Including a high level of ability with Microsoft Office applications (MS Word, Excel, Outlook).
· Must be flexible to accommodate the changing needs of the clinic.
· Reliable transportation to fulfill the essential functions of the position.
