Position: Direct Support Professional

Trinity DTA upholds a team-oriented ethos, leveraging the diverse educational backgrounds and specialized skills of staff members to deliver optimal care to client members. Each person on the team is esteemed for their individual contributions and the transformative influence they bring to those in their care.
Location: Phoenix, Arizona
Hours: 40/week, Monday through Friday, 8:30am-4:30pm
As a Direct Support Professional working with members in Trinity DTA, you can expect a typical day to include:
- Assist members in recreational, social and community activities based on their preferences and their person-centered service plan (PCSP)
- Review and implement the PCSP and behavioral plan goals (BTP)
- Maintain daily and weekly case notes and daily objectives tracking sheets to increase independence
- Encourage members in establishing social networks, promote participation in community outings and volunteer events
- Provide transportation to and from activities
- Model behavior by using appropriate language, manners, and dressing appropriately
- Adhere to confidentiality policy according to state and federal law
- Help with fun activities that address goals (e.g. games to work on taking turns, going on walks for fitness, etc.).
- Document progress to share with families
- Working as part of a team to ensure a safe, fun environment
- Field trips
- Other duties as assigned

Minimum Qualifications:
- Must be at least 18 years of age
- Valid AZ Driver's License for driving company vans
- Must have a clean 39-month motor vehicle driving record
- Pre-employment background check (Adult Protective Service, Department of Child Safety, OIG, SAM)
- High school diploma or equivalent work experience
- Experience and ability to work with challenging behaviors
Ability to Obtain:
- Class One Fingerprint Clearance Card
- CPR /Basic Life Support/First Aid Certification
- Article 9
- Prevention and Support Certification
- Behavior Management
- Recognizing Neglect, Abuse & Exploitation Certification
Knowledge, Skills and Abilities:
- Demonstrate organizational skills and strong attention to detail
- Proficient communication skills (verbal/written) and interpersonal skills
- Knowledge and understanding of developmental/cognitive and/or physical disabilities, behavior modification skills, and intervention strategies that can be implemented
- Ability to function both independently and as part of a team
- Ability to work under stress
- Ability to plan, learn new programs, have excellent follow through, problem-solving skills
- Maintain a positive attitude and collaborate with members, staff, and families
- Basic computer skills in programs such as Microsoft Outlook, Teams, Word
- Demonstrate understanding of the challenges faced by disabled individuals
- Ability to communicate information and concerns about members to the DTA Program Supervisor
Pay: $15.75 for driving responsibilities and no credentials
Pay: $15.25 for non-driving responsibilities and no credentials
Once credentials are received and validated, a pay increase of 25 cents will be in effect.
*Completion of training is required prior to member interaction*

Pay: $15.75 - $16.00 per hour
