Job Description:
We are seeking a dedicated and reliable Medication Management Registered Nurse (RN) to join our team. The RN will visit patients at their homes once per week to refill pill boxes and discuss medications with the patients. Consistency is key, so the same nurse is required to commit to at least four weeks with each patient.
Responsibilities:
- Visit patients in their homes once per week to refill their medication pill boxes.
- Review and discuss each patient's medication regimen, ensuring they understand their prescriptions and the importance of adherence.
- Monitor for any potential side effects or interactions and report any concerns to the patient's primary care provider.
- Provide education on medication management, storage, and administration.
- Maintain accurate and detailed records of each visit and the services provided.
- Develop a trusting and supportive relationship with each patient to encourage open communication about their health and medications.
- Commit to a minimum of four weeks with each assigned patient to ensure continuity of care.
Qualifications:
- Active Registered Nurse (RN) license in the state of CA
- Minimum of 5 years of nursing experience, preferably in a home health, geriatric, or chronic disease management setting.
- Strong knowledge of pharmacology and medication management.
- Excellent communication and interpersonal skills, with the ability to educate and support patients effectively.
- Detail-oriented with strong organizational and documentation skills.
- Ability to work independently and manage time efficiently.
- Reliable transportation and willingness to travel to patients' homes.

Pay: $85.00 - $90.00 per hour
