Job Summary: Supervises/assists with coordination of all aspects of health care for the individuals served within Rowley Bridge Road in collaboration with the treatment team. Ensures individuals served achieve their maximum level of health and wellness. Supervises overall clinical operations of the residential home.
Schedule:
Monday – 9AM – 5PM through Friday - 9AM-5PM
Pay:
$43 - $47/hr per hour, depending on experience
Primary Responsibilities:
- Demonstrate knowledge of and implement Northeast Arc Policy and Procedures, including the Code of Conduct, Residential Division Standards, and Funding Source Regulations.
- Utilize a PBS approach when working with staff and individuals, including promoting the agency objectives related to personal growth, community involvement, and decision-making.
- RN Case Manager is responsible to communicate and collaborate with Program Directors, to obtain and review medical records, current medications, health status and functional abilities consistent with ISP and Annual Physical
- Responsible for developing communication and collaborative relationships with all medical and Ancillary Services involved in the individual’s care.
- Responsible to act as an advocate and liaison for the individual to assure that they are receiving a level of care consistent with clinical best practices.
Case Management Responsibilities:
- Conducting a comprehensive nursing assessment of the individual’s health and psychosocial needs and develop a Case Management Plan collaboratively with the individual, family and caregivers
- Develop a Health History for each individual admitted to the Residential Division
- Complete DDS Health Care Record and Health and Dental within ISP timelines.
- Medical Plan of Care and Nursing Care Plan are maintained and modified quarterly.
- Maintain documentation of health care encounters, case management communication and monthly key data in the Section three of the Health Care fact sheet
- Episodic Case Management for hospital admissions and discharges.
Requirements:
- Must be fully licensed as a Registered Nurse in Massachusetts and must maintain licensure from the Massachusetts Board of Registration in Nursing. Minimum three years nursing experience and three years’ experience in the direct care of the elderly or disabled adults is required.
- Must have the eligibility to become a MAP Coordinator
- Must maintain CPR certification and complete required programmatic trainings.
- Must drive to various locations for home visiting purposes and medical appointments.
- Light physical effort might be required.
