The Intake Specialist Case Manager is responsible for assisting our clients with intake documents to obtain services
for individuals on the CCC+, CL and FIS waivers as well as EPSDT program recipients to ensure
their safety, satisfaction, health, and welfare while living independently with supports in the
community. Full time (40 hours a week) non-exempt.
Must Live in the Eastern Time Zone (EST).
Requirements
Skills and Qualifications include:
- Requires a documented combination of skills/relevant work experience. Person-
- Centered Thinking/Planning training a plus.
- Work remotely with electronic devices and associated tools and equipment required to complete day to day tasks.
- Maintains experience in a variety of applications and programs; not limited to but including Office, Google, & Adobe.
- Must maintain a steady and reliable internet connection.
- Have good written and verbal communication skills.
- Submit to criminal background check.
- Two references are required.
Professional Responsibilities
- Makes sure all necessary documents and paperwork are correct and turned in by specified deadlines.
- Meets all established KPI's
- Follows all HIPAA guidelines with personal information and client records. This includes using secure email when communicating in writing.
- Responds to all voice mails and emails within 1 business day.
- Consistently attends all required meetings.
- Consistently completes all training requirements. .
- Utilizes team support tools available to him/her
- Understands that as a Moms in Motion team member, he/she is a state mandated reporter of suspected/observed/reported abuse or neglect of an individual. Alerts supervisor of issues and fully and accurately utilizes the Abuse/Neglect Reporting Tab in WISE.
- Manages dashboard appropriately, runs reports, and takes action on necessary items.
