Full time RN / Population Health Care Coordinator needed for one of the top performing Accountable Care Organizations in the United States. Essential Duties and Responsibilities:
- Design, implementation and management of population-based care coordination activities.
- Develop, implement, and evaluate clinical initiatives to improve patient care.
- Monitor patient outcomes and analyze data to measure the effectiveness of clinical initiatives.
- Serve as a resource for evidenced based practice and clinical guidelines.
- Work collaboratively with physicians, staff and other health care professionals utilizing the latest technology to provide improved quality of care while reducing unnecessary tests, procedures, and overall costs.
- Assessing / analyzing advanced reporting and data metrics to determine the healthcare, educational and psychosocial needs of the patient and their family.
- Designing and monitoring an individualized plan of care with the patient that will close gaps in care.
- Facilitates care for high-risk members utilizing appropriate physician services (primary / specialist), care transitions and coordination of community support services.
- Develop processes and procedures that will comply with all care coordination contract requirements.
- Responsible for developing strategies/toolsets for more accurate, efficient and engaging physician member, key physician and office staff communication.
- Patient Engagement Systems: Must be able to work efficiently in patient engagement processes and systems including: Software management systems / reporting systems / SMS texting / Newsletters / patient outreach initiatives (i.e. wound care initiates) health fairs and other local promotions.
- Building systems that will create lasting relationships with both patients and physicians.
- Submits files /documentation to physicians and other clinicians as requested for review, quality assurance checks, and other purposes.
- Developing a team approach by working collaboratively with the primary care provider and other members of the healthcare team to ensure coordination of services.
- Implementing clinical interventions based on evidence based clinical guidelines.
- Consulting / training and education on-site for physician offices.
- Must abide to all HIPAA, Confidentiality and Privacy laws.
- Other duties may be assigned by your Supervisor and Managers.
Competencies / Qualifications:
To perform the job successfully, an individual should demonstrate the following competencies:
- Must have a minimum RN – BSN; Master of Science in Nursing (MSN) preferred.
- 2+ years Patient Engagement / Interaction
- Leadership and Project Management skills
- Ability to work independently and as part of a multidisciplinary team.
- In-depth knowledge of health delivery systems /general practices
- Advanced skills with Spreadsheets / Report Writing / PowerPoint / Word
- Experience with intranet or web-based communication vehicles
- Excellent written and verbal communication skills
- Oral Communication - Speaks clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions.
- Safety and Security - Uses equipment and materials properly.
- Attendance/Punctuality - Is consistently at work and on time.
- Knowledge of HIPPA: Legal and ethical consideration related to employee information.
- Must be computer literate in EMR and Microsoft office products
- Some after hours on call responsibility
- Unrestricted driver's license
- Experience with data analysis and quality improvement methodologies is a plus.
