Targeted Case Manager (TCM)
Job Description and Profile
The Targeted Case Manager (TCM) for Rural Nevada Counseling (RNC), is an integral member of its Certified Community Behavioral Health Center (CCBHC), providing exemplary targeted case management services to its clients. The TCM assists the clients to gain access to any needed resources for medical, social, education, and other supportive services including housing and transportation needs. The TCM’s role does not include the direct delivery of medical or clinical patient services. Components of TCM services include case management assessment, care planning, referral, linkage, and monitoring follow-up. All TCM service providers must comply with Medicaid Services Manual (MSM), Chapter 2500, Case Management. Target groups for the RNC’s CCBHC include those listed under MSM Chapter 2500 as follows: Non-Seriously Mentally Ill (Non-SMI) Adults, Serious Mental Illness Adult (SMI), Non-Severely Emotionally Disturbed (Non-SED Children and Adolescents), and Severely Emotionally Disturbed (SED) Children and Adolescents. The TCM links clients to community resources to address social determinates of health, such as access to safe housing, food, and basic health care. When the TCM is working with children and their families, community resources must also be leveraged to provide wrap-around supports to increase family resiliency and reduce the risk of further crisis. The TCM is a qualified individual who possesses the skills and abilities to work collaboratively with and under the Clinical Director and the TCM Supervisor.

Duties and Responsibilities
- Assessment of the eligible individual to determine service needs
- Development of a person-centered care plan
- Referral and related activities to help the individual obtain needed services
- Monitoring and follow-up with clients treatment
- Assessment and periodic reassessment of individual’s needs, to determine the need for any medical, educational, social, or other services
- Identifying the needs of the individual and completing related documentation
- Gathering information from other sources, such as family members, medical providers, social workers, and educators (if necessary) to form a complete assessment of the eligible recipient
- Development (and periodic revision) of a specific care plan based on the information collected through the assessment
- Specify the goals and actions to address the medical, social, educational, and other services needed by the eligible recipient
- Includes activities such as ensuring the active participation of the eligible recipient and working with the recipient (or the individual's authorized health care decision-maker) and others to develop those goals
- Works closely with primary Clinician and care team of client’s to ensure client’s individual needs are being met.
- Identifies a course of action to respond to the assessed needs of the eligible recipient
- Referral and related activities (such as scheduling appointments for the recipient) to help the eligible individual obtain needed services, including activities that help link the individual with medical, social, and educational providers or other programs and services that can provide needed services to address identified needs and achieve goals specified in the care plan
- Monitoring and follow-up include activities and contacts that are necessary to ensure that the care plan is effectively implemented and adequately addresses the needs of the eligible individual and may be with the individual, family members, service provider or other entities or individuals.
- Services are being furnished in accordance with the individual's care plan
- Services in the care plan are adequate
- Monitoring if there are changes in the needs or status of the eligible client
- Monitoring and follow-up activities include making necessary adjustments in the care plan and service arrangements with providers. Monitoring may involve either face-to-face or telephone contact, at least annually.
- Works within the scope of practice and competency as well as document client’s progress and interventions appropriately
- Maintain professional and personal boundaries
- Ability to establish and maintain effective working relationships at all levels of the organization, including management, co-workers, clients, vendors, and associations
- Understands State and Federal laws regarding privacy confidentiality, and security along with mandated reporting
- Able to maintain confidentiality of sensitive documents and information
- Exemplary customer service skills, including ability to create positive first impressions and develop partnerships internally and externally
- Excellent interpersonal and strong oral and written communication skills
- Excellent organizational and multi-tasking skills, including strong attention to detail
- Able to work on multiple projects and effectively manage changing priorities
- Proficiency in software applications, using Microsoft Office (Outlook, Word, Excel, Access & PowerPoint)
- Effectively provide verbal and written communications on behalf of the client to all stakeholders.
- Participates in case presentations, evaluations, and staff meetings
- Bills only for services provided
- Other duties, responsibilities, and assignments as needed
Education, Qualifications and Experience
- Must be a citizen of the United States or lawfully entitled to remain and work in the United States –proof of citizenship or proof that an applicant is able to work in the United States
- Must pass FBI Criminal background check
- Must Pass any drug testing as required by the Agency
- Must be over 21 years of age
- Drivers’ History Report with no major violations
- Ability to work collaboratively with colleagues and outside agencies
- Proficiency in software applications, using Microsoft Office (Outlook, Word, Excel, Access & PowerPoint)
- Excellent organizational and problem-solving skills
- If in recovery, candidate must have a minimum of 2 years of sobriety
- Minimum Bachelor’s degree in a health-related field, Registered Nurse (RN), master’s level professional (LCSW or LMFT), Advanced Practice Registered Nurse (APRN) in mental health, psychologist or mental health professional who works under the direct supervision of the TCM Supervisor and Clinical Director
Job Type: Full-time
Pay: $20.00 - $27.00 per hour
