Central Alabama Aging Consortium (CAAC) is currently seeking a socially minded individual to join our team as a Case Manager. Our mission at CAAC is to develop, coordinate, and provide services in the community that supports an active and independent lifestyle where those we serve can live and thrive.
Case Managers with CAAC:
- Conduct Assessments and monitor services for the elderly, disabled and homebound
- Develop person centered care plans and monitor client's progress
- Collect data and enter reporting information
- Develop and maintain general knowledge of the Medicaid Waiver program
Requirements:
- Bachelor's degree in nursing, social work or related field
- Ability to travel within the tri-county area (company car provided)
- Good computer skills
- Good communication skills
