Child Case Manager/ High Fidelity Wraparound Case Manager - Summary
NorthKey Community Care is seeking a full-time High Fidelity Wraparound Case Manager to work collaboratively with families, their supports, other professionals, and community partners to develop individualized, family determined, strength-based plans of care. This will occur through regular contact with all team members, and monthly family team meetings. A Bachelor’s Degree in a human services field is required.

High Fidelity Wraparound Case Manager - Qualifications
- Bachelor's degree in an acceptable human services or behavioral health field of study from an accredited school is required.
- Experience working in the behavioral health and/or mental health field for at least one year.
- Relevant knowledge, training, and experience in the behavioral health and/or mental health field.
- Excellent written and verbal communication skills.
- Ability to create effective relationships with individuals, family members, and community partners.
- Excellent time management skills.
- Ability to work independently while also working effectively in a team environment.
- Ability to manage conflict effectively within a team environment.
- May 2024 graduates encouraged to apply.
High Fidelity Wraparound Case Manager - Responsibilities
- Establish rapport with families, their supports, other professionals, and community partners, and continue strong relationships throughout the process with all parties.
- Deliver wraparound services in a manner that respects family culture, supports family voice and choice, and maintains a strength-based focus throughout the High Fidelity Wraparound process.
- Complete Targeted Case Management certification training, as well as all High Fidelity Wraparound trainings including but not limited to the following: Introduction to Wraparound, Engagement, Intermediate and ongoing peer meetings (as scheduled). Other fidelity measures include weekly supervisions, observations and recordings of supervisions and team meetings.
- Complete comprehensive strengths-based assessment of the individual and family, working in full partnership with team members to develop and implement a plan of care, identifying providers of services and natural supports, facilitating team meetings, making regular home visits, monitoring all services, and maintaining fiscal accountability.
- Offer consultation and education to all providers regarding the values of the wraparound process, monitors progress toward the goals, and assures that all necessary data for evaluation are gathered and entered.
- Comply with all Wraparound, NorthKey, and Medicaid guidelines, which include detailed documentation of progress, timely and accurate documentation, home and school visits with families, and addressing progress and barriers toward their goals.
- Demonstrate dependability in work habits, judgment, and adherence to ethical standards.
- Attend scheduled staff meetings, supervisions, trainings, conferences, and in-service trainings as expected, including maintaining applicable certifications.
- Become familiar with, and maintain regular contact with, various recreational activities, educational resources, and support programs in the community.
