We are seeking a Hospice RN to join our team! You will be responsible for providing skilled nursing care to terminally ill patients and families based on the systematic assessment of health needs with supervision by the primary physician. Participate in on-call rotation.
Responsibilities:
Understand the philosophy and mission of hospice and palliative care
Autonomously provide skilled nursing care to patients based on an understanding of end stage disease process and palliative symptom management.
Establish and coordinate treatment plans with patients & families from the admission process throughout end of life.
Maintain up-to-date records so the problems, plans, actions and goals are accurately and clearly stated
Monitor, manage and report changes in patient symptoms or behaviors. Triage and prioritize patient and family needs.
Teach families and other primary care persons appropriate care techniques
Provide emotional support to patient and family
Maintain regular communication with the interdisciplinary team, attending physician, medical director and clinical director concerning patients and families
Maintain accurate patient medical records
Complete performance improvement activities, as assigned
Qualifications:
Previous experience in nursing (Hospice, HH, LTC are beneficial but not required)
Familiarity with medical software and equipment
Ability to build rapport with patients/families
Strong problem solving and critical thinking skills
Ability to thrive in a fast-paced environment

Pay: From $69,440.00 per year
