Minimum Qualifications:
- Bachelor's Degree in Social Work, Counseling or Psychology
- CADC/CAADC or the Ability to obtain a McBap Development Plan
Scheduled Hours Worked: Monday-Friday 4:45am-2:15pm
Summary: Works independently to complete initial assessments for individuals who are seeking outpatient treatment. Responsible to efficiently assess the needs of new clients, and appropriately connect them with both internal and external resources.
Responsibilities:
- Conducts screenings and assessments with clients to evaluate the nature of their presenting problems in order to determine their needs and develop appropriate service plan goals.
- Develops and provides initial individual service plans; schedules and/or arranges appropriate ongoing treatment according to the behavioral health recipient’s needs and wishes, provides internal and external resources.
- Prepares intake documentation, progress notes, event notifications and other documentation in accordance with regulatory requirements and company policies and procedures.
- Ensures required documentation is completed in accordance with established timelines; including, but not limited to, screenings, assessments and treatment plans.
- Facilitates coordination of care, to include: notification to program staff of new behavioral health recipient intake, summary of intake information and recommended services/programs.
- Responsible to explain the evaluation process, sets up an appointment for the prospective patient to meet with the counselor and answers any questions the client may have.
- Participates in clinical staff meetings, trainings and other activities as required.
