Optimal Hospice is looking for a passionate and seasoned Registered Nurse Case Manager (RNCM) to join the team. The Registered Nurse, RN will provide care to patients at their home or placement. Mileage will be reimbursed.

THE OPTIMAL HOSPICE REGISTERED NURSE:

Hospice heart: compassionate, kind and empathetic to your patients and their families

Nurse/Patient Relationship: Develops and maintains a therapeutic relationship through the health care continuum

Provide direct patient care services to hospice patients

Empower patient's caregivers by education them on care for their loved one

Provide coordination of care, ensure continuous assessment of each patient's and family's needs

Responsible for documentation and communication of care, medication, and observations made during care of the patient in order to track responsiveness and health of patient.

Responsible for providing appropriate patient/family education to help patient/family achieve a pain-free and comfortable status.

Weekend on-call rotation (additional pay)
PAY: $88,000-100,000 DOE
About YOU:
-
Registered nurse with current licensure to practice professional nursing in the state.

Graduate of National League for Nursing accredited school of nursing.
-
Maintains a current CPR certification.
-
Current valid driver’s license.
-
At least one year of Hospice experience (preferred)
-
Demonstrates excellent observation, problem solving, verbal and written communications, nursing skills per competency checklist.

Shows ability to organize and prioritize workload independently.
