Responsibilities:
- Coordinating total patient care by conducting comprehensive health and psychosocial evaluations, monitoring patients' conditions, promoting sound preventive practices, coordinating services and performing teaching and training activities.
- Evaluating effectiveness of nursing services to patients and families
- Consulting with attending physicians for change-of-Care Plans, and coordinating care accordingly with the Director of Nursing.
- Participating in development and periodic revisions of physicians' Plans of Treatment, and processing change orders as needed.
- Covering and filling in for other nurses as assigned by Director of Nursing.
- Providing professional, consistent, skilled nursing care as outlined in nursing care plan.
Minimum Qualifications:
- RN/LPN license required.
- Demonstrated competence in interpersonal relationships, verbal and written communication.
- Skills in Microsoft Office and EMR software.
