Job Summary:
We are seeking a skilled Registered Nurse to join our healthcare team. The ideal candidate will have expertise in wound care. Under the direction of the Clinical Coordinator and Program Director, the Wound Care RN will perform the duties of patient care. This person is responsible for treatment, safety, patient care coordination, quality management, and patient, professional, and community education. This RN is also responsible for coordinating clinical services and assuring consistent quality care for patients in the program.
**Responsibilities:**
- Provide wound care to in-patient and one outpatient center
- Manage acute and chronic medical conditions effectively
- Utilize electronic health record systems for documentation
- Collaborate with multidisciplinary teams to ensure optimal patient outcomes
- Serve as a consultant for the treatment of patients with altered skin integrity.
- Uses critical thinking skills and sound judgment based on evidence based practices when making decisions for appropriate wound care treatment.
- Conducts in-services and updates on wound and ostomy care as requested to Nursing and ancillary departments.
- Participate in continuing education by attending or completing courses which deal with wound care.
- Recognizes changes in a patient condition and communicates effectively with the team and physician to adjust treatment plans.
- Keep current with new products or technologies in WOC nursing.
**Requirements:**
- Valid RN license in the state of practice.
- Certification in wound care, acute care or primary care specialty.
- Minimum of one or more years of wound care, acute care medical-surgical experience desirable. Prior leadership and/or teaching experience preferred.
- Ability to commute and travel to patients.
- Strong understanding of similar EHR platforms.
- Demonstrated proficiency and knowledge of current ostomy, wound and incontinence management.
- Excellent interpersonal and communication skills for patient care.
**License/Registrations/Certifications**
- Current Active California Registered Nurse License
- Certified Wound, Ostomy, Continence Nurse Certification (CWOCN) current or within 1 year of hire
- Current American Heart Association BLS certification
