Job Summary: M&N HOME CARE is seeking a top tier CDPAP Intake Coordinator for our Bronx office. The perfect candidate will understand the regulations governing the home care field, related to Medicare, Medicaid and other insurances, and is committed to excellence in patient care. The Intake Coordinator is responsible for managing the initial client or patient intake process, ensuring accurate and timely collection of information, and providing exceptional customer service. This role serves as the first point of contact for individuals seeking services, coordinating their entry into the system, and facilitating communication between clients, service providers, and internal departments.
Key Responsibilities:
- Client Intake and Assessment:
- Conduct initial interviews with clients/patients to gather comprehensive information about their needs and eligibility for services.
- Accurately document client information, medical history, and service requirements into the database.
- Evaluate client needs and refer them to the appropriate services or departments.
- Coordination and Scheduling:
- Schedule appointments and coordinate with relevant departments to ensure clients receive timely services.
- Maintain and update scheduling systems to reflect current client appointments and service statuses.
- Communicate appointment details and any necessary preparations to clients.
- Documentation and Record Keeping:
- Ensure all client records are accurately maintained and updated in compliance with company policies and regulations.
- Prepare and manage intake forms, consent forms, and other necessary documentation.
- Safeguard client confidentiality and handle sensitive information with discretion.
- Customer Service:
- Provide courteous and empathetic customer service to clients, addressing their questions and concerns.
- Assist clients in understanding the services available to them and guide them through the intake process.
- Follow up with clients to ensure their needs are being met and to gather feedback on their service experience.
- Collaboration and Communication:
- Collaborate with internal departments and external agencies to facilitate seamless client transitions and referrals.
- Communicate effectively with service providers to ensure client needs are understood and addressed.
- Participate in team meetings and contribute to process improvement initiatives.
- Compliance and Quality Assurance:
- Adhere to all organizational policies, procedures, and regulatory requirements.
- Participate in training and professional development to stay current with industry standards and best practices.
- Contribute to quality assurance efforts by identifying areas for improvement in the intake process.
Qualifications:

- Experience:
- Minimum of 2 years of experience in Home care (Preferred)
- Minimum of 2 years of Intake (Preferred)
- 2 years in Customer service, healthcare, or social services environment, with direct experience in intake coordination preferred.
- Skills:
- Strong organizational and time-management skills.
- Excellent verbal and written communication skills.
- Proficiency in Microsoft Office Suite and experience with database management.
- Ability to handle sensitive information with discretion and maintain confidentiality.
- Empathetic and patient-focused approach.
- Experience working with the HHA Exchange Platform is preferred
- Track record of handling sensitive and confidential information with the utmost professionalism
- Strong customer service orientation.
- Education:
- High school diploma required; Bachelor's degree or equivalent professional experience preferred.
