Role Overview: As an MDS Coordinator at Venza Care, you will be responsible for the coordination and completion of the Minimum Data Set (MDS) and care planning, ensuring compliance with federal and state regulations. Your role will directly influence the quality of care provided to our residents and the overall success of our healthcare services.
Responsibilities:
- Conduct comprehensive assessments of new and existing residents using the MDS tool.
- Develop, implement, and review individualized care plans in collaboration with an interdisciplinary team.
- Ensure the timely transmission of MDS assessments to the relevant authorities.
- Train and guide nursing staff on MDS procedures and updates.
- Monitor and audit clinical records for accuracy and completeness.
- Collaborate with healthcare providers to optimize patient care and outcomes.
Qualifications:
- **Active RN license with MDS Certification preferred.**
- Proven experience in clinical assessment and MDS coordination.
- In-depth knowledge of federal and state regulations pertaining to MDS and care planning.
- Strong organizational skills with attention to detail.
- Excellent communication and interpersonal skills to interact with staff at all levels.
