A rapidly growing Licensed Home Care Services Agency (LHCSA) In Brooklyn is looking to add a motivated and enthusiastic Scheduling Coordinator to its growing staff. We are currently looking for the right candidate to work a full time position in our Corporate location. A Home Care Coordinator is responsible for the organization and coordination of schedules between patients and their caregivers. This self-starting individual needs to understand the importance of completing tasks and staying organized, and providing valuable customer support to patients or caregivers.
Requirements:
- Must be able to handle multiple and consistent incoming call.
- The ability to multi task and work at an extremely fast pace
- Customer service Skills
- Home Care Scheduling Experience a must
- Knowledge in HHA Exchange a must
- Bilingual a must
- Weekday/Weekend On Call REQUIRED
Responsibilities:
- Do daily attendance
- Request EVV or timesheets
- Ensures aide placement specific to Patient’s needs and request.
- Ensures aides understand and can fulfill the Plan of Care.
- Reviews temp orders daily. Update and document in HHA Exchange.
- Existing patients must be called once a month using the communication template and documented in HHA Exchange.

Pay: $20.00 - $22.00 per hour
