Job description
Concordia Hospice of Washington is seeking a Clinical Supervisor, RN for a full time position which will report into our Administrative office in Washington, PA.
The Hospice Clinical Supervisor is a licensed Registered Nurse responsible for overseeing day to day patient care by assisting patients, caregivers/families, Interdisciplinary Group (IDG) members and health care providers involved in patient care. The Clinical Supervisor, RN is a supervisory role to ensure patient care is coordinated and managed efficiently and with the highest quality in accordance with state and federal guidelines.
QUALIFICATIONS for Clinical Supervisor Registered Nurse:
- Current Registered Nurse (RN) license in the state of Pennsylvania
- Two (2) years hospice experience preferred
- Possesses strong assessment, problem solving and decision making skills
- Solid computer skills, including Word, Excel and electronic medical record (EMR) programs
- Excellent organizational skills with attention to detail
- Outstanding communication skills (both written and verbal)Must be able to identify and anticipate patient and family needs
- Must possess the ability to coordinate with scheduling sufficient patient coverage on evenings, weekends and during vacations and holidays
